Viral reactivation in the lungs of patients with severe pneumonia is associated with increased mortality, a multicenter, retrospective study

Abstract: Purpose: This study aims to assess the clinical features of virome in the early onset of severe pneumonia in ICU patients. Methods: A total of 97 patients' clinical data was collected retrospectively from intensive care units of ve teaching hospitals in China from June 2018 to July 2021 followed by metagenomic next-generation sequencing (mNGS) of the bronchoalveolar lavage uid (BALF) at the onset of severe pneumonia. Results: Cytomegalovirus (CMV), herpes simplex virus-1 (HSV-1), and Epstein-Barr virus (EBV) wer… Show more

“…The patient’s peripheral blood did not reveal pathogens other than parvovirus B19 detected by metagenomic next-generation sequencing as described before ( 27 , 28 ). Also, whole exome sequencing did not identify any HLH-associated mutations.…”

“…However, results from observational studies suggest that infection with a PVL‐positive S. aureus does not predict a poor clinical outcome for pneumonia 3 . Additionally, Herpesvirus is the most common reactivated virus in the lower respiratory tract of patients with severe pneumonia, and the incidence of viral reactivation increases with a longer disease duration 4 . Therefore, testing positive for Herpesvirus is associated with viral reactivation later in the course of the disease.…”

“…No clear relationship has been shown between the development of HHVs reactivations and deleterious consequences regarding morbidity (prolonged mechanical ventilation, bacterial superinfection and/or hospital stay) and mortality. Therefore, there is no definitive answer to the question whether viral reactivation is only one of the hallmarks of the severity of an underlying disease or it is directly responsible for organ disease such as pneumonia [ 1 4 ] and colitis [ 5 ] .…”
