Youngest case of ductal carcinoma in situ arising within a benign phyllodes tumour: A case report

Abstract: HighlightsPhyllodes tumour is a rare type of breast tumour, accounting for less than 1% of benign and malignant breast tumours.Phyllodes tumour is classified pathologically as benign, borderline or malignant.Phyllodes tumour with ductal carcinoma in situ can exist as separate foci or arise from within the tumour.Phyllodes tumour usually presents in late 50s but has never been reported in a 23-year-old nulliparous woman.

“…Some key histological factors are required to make the diagnosis of phyllodes tumors. Factors such as stromal overgrowth, hypercellularity, atypia, high mitosis rate, infiltrative borders and the presence of necrosis [1,2,4,6], are necessary to distinguish the tumor between benign and malignant phyllodes. Although, they have a structural similarity with its benign relative--the fibroadenoma, cystosarcoma phyllodes have more significant hypercellularity and overgrowth of the stromal protrusion.…”

“…Phyllodes tumors are further divided into three groups of benign, borderline or malignant tumors according to the World Health Organization (WHO) 2012 classification [1,2,4,[6], [7], [8],12]. The diagnostic criteria are also based on similar criteria for the initial diagnosis: stromal overgrowth, hypercellularity, atypia, high mitosis rate, infiltrative borders and the presence of necrosis.…”

“…However, among the ten cases, only six had pure DCIS from benign phyllodes tumours, ours being the seventh case to be reported in the English literature. 5,15,19,33,36,39 The size of the phyllodes tumours ranged widely between 2.0 cm and 21.0 cm, suggesting that DCIS can occur in any palpable phyllodes tumour. Eight cases underwent wide local excision while the rest underwent total mastectomy.…”

“…It is recommended that the treatment be customized in each case based on whether there is an invasive component, affected lymph nodes or distant metastases, and that the carcinomatous component be treated independently of the PT (8,9). Axillary lymph node dissection is not part of the standard treatment for phyllodes tumors as lymph node spread is rare.…”
