BackgroundAccurate early diagnosis and prompt treatment is one of the key strategies to control and prevent malaria in Ethiopia where both Plasmodium falciparum and Plasmodium vivax are sympatric and require different treatment regimens. Microscopy is the standard for malaria diagnosis at the health centres and hospitals whereas rapid diagnostic tests are used at community-level health posts. The current study was designed to assess malaria microscopy capacity of health facilities in Oromia Regional State and Dire Dawa Administrative City, Ethiopia.MethodsA descriptive cross-sectional study was conducted from February to April 2011 in 122 health facilities, where health professionals were interviewed using a pre-tested, standardized assessment tool and facilities’ laboratory practices were assessed by direct observation.ResultsOf the 122 assessed facilities, 104 (85%) were health centres and 18 (15%) were hospitals. Out of 94 health facilities reportedly performing blood films, only 34 (36%) used both thin and thick smears for malaria diagnosis. The quality of stained slides was graded in 66 health facilities as excellent, good and poor quality in 11(17%), 31 (47%) and 24 (36%) respectively. Quality assurance guidelines and malaria microscopy standard operating procedures were found in only 13 (11%) facilities and 12 (10%) had involved in external quality assessment activities, and 32 (26%) had supportive supervision within six months of the survey. Only seven (6%) facilities reported at least one staff’s participation in malaria microscopy refresher training during the previous 12 months. Although most facilities, 96 (79%), had binocular microscopes, only eight (7%) had the necessary reagents and supplies to perform malaria microscopy. Treatment guidelines for malaria were available in only 38 (31%) of the surveyed facilities. Febrile patients with negative malaria laboratory test results were managed with artemether-lumefantrine or chloroquine in 51% (53/104) of assessed health facilities.ConclusionsThe current study indicated that most of the health facilities had basic infrastructure and equipment to perform malaria laboratory diagnosis but with significant gaps in continuous laboratory supplies and reagents, and lack of training and supportive supervision. Overcoming these gaps will be critical to ensure that malaria laboratory diagnosis is of high-quality for better patient management.
Wise utilization of water resources is becoming very important as world faces water crises. The main objective of this study was to investigate the rural water supply systems with case study in Adama area, in central Ethiopia. Both quantitative and qualitative data were collected and analyzed. Four sample water schemes were selected and totally 148 (63 were female) representative households were selected for answering the questionnaires. Key informant interviews and group discussions were also conducted. The study assessed issues such as community participation, water committee empowerment, management and governance of water supply schemes, women participation, functional status of water supply scheme, sanitation and hygiene issues, external support, and monitoring system of water supply schemes. The findings indicated that the community participation in planning and implementation was very good while monitoring mechanism of operation and management as well as community participation on choice of technology was poor. The water schemes were located at reasonable distances i.e. less than 2 km in most cases and the time taken for round trip to fetch water from source was less than or equal to 30 minutes in most cases, however the queuing time was more than an hour. The water supply was inadequate as only about 15% of beneficiaries could get 20 liters of water per day per capita. The water sources were exposed in many cases to human waste, wild life, livestock and uncontrolled flooding. Sanitary practices in the study area were poor as only about 3.4% had ventilated and improved pit latrine and open pit and/or open field defecation were widely practiced.
Purpose Traditionally prepared enset-based products like bulla are characterised by low protein and vitamin contents and are highly viscous; this causes protein-energy malnutrition especially in infants and young children. This paper aims to improve the nutritional and sensory qualities of enset-based food with pumpkin and amaranth. Design/methodology/approach Sixteen formulations of composite flour were generated using a D-optimal constrained mixture design with a range of 50-80 per cent bulla, 10-25 per cent pumpkin and 10-40 per cent amaranth flour. Nutritional and sensory qualities of the formulations were investigated using standard methods. Findings Ash, fat and protein contents increased from 1.1-2.8 g/100 g (p < 0.001), 1.7-3.9 g/100 g (p < 0.001) and 6.1-9.0 g/100 g (p < 0.001) respectively with the increased amaranth flour. Crude fibre content increased with increasing percentage of the amaranth and pumpkin flours. On the other hand, carbohydrate content increased from 78.2 to 84.3 per cent (p < 0.001) with an increase of bulla flour. A significant increase in Fe, Zn and Ca from 6.8-10.5 (p < 0.01), 1.5-3.0 (p = 0.022) and 1.8-148.5 mg/g (p < 0.001) respectively was achieved with an increase in amaranth ratio in flour. The total carotenoid content increased with increase in the percentage of pumpkin flour. The best sensory attributes of the developed product were around the centre point of the three ingredients. Originality/value The study showed that a product with good nutritional value and sensory acceptability could be prepared from enset-based ingredients and the optimal product was obtained by blending 57.8 per cent bulla, 18.9 per cent pumpkin and 23.3 per cent amaranth flour. The enriched flour can have application for snack food preparation.
Maize is the second most widely grown cereal and gaining importance as a highly nutritious crop in Ethiopia. However, it is severely destroyed by storage insect pests and needs further research to minimize losses. In line with this, research was initiated to evaluate the efficacy of two botanical plant powders (Eucalyptus globulus Labill leaf and Chenopodium ambrosioides L. whole plant) against storage insect pests of maize grains of two maize varieties (BH-661 and Limu) in polypropylene sacks storage conditions at Jimma Zone, Sokoru district. The plant powders were compared with untreated control, and completely randomized design was used in the experiment with three replications for each treatment. Germination capacity, thousand grain weights, percent of insect damage, and weight loss of the stored grains were evaluated and reported in the range of 69.67–94.33%, 318.7–339.3 g, 3.67–50%, and 0.2843–5.22%, respectively, after five months of storage for grains treated with botanicals. However, germination capacity of 10% and 65.33%, percent insect damage of 80.33% and 48%, and weight loss of 23.53% and 5.89% were observed for BH-661 and Limu varieties, respectively, after five months of storage for untreated control. The result indicated that both tested botanicals were effective in protecting the storage insect pests and maintaining the quality of the grains tested in comparison with control and Chenopodium ambrosioides L. whole plant powder is more effective. Although there was significant protective effect compared to untreated control, their effectiveness was decreased drastically after five and three months of storage for Chenopodium ambrosioides L. whole plant powder and Eucalyptus globulus Labill leaf powder, respectively. It is recommended that further research should be done to check if the increasing rate of application increases protection duration of these botanicals and the toxicity of Chenopodium ambrosioides L. should be further studied to use it as a storage insect protectant of maize grains intended for food purpose.
