RESUMO: Objetivou-se identificar atividades dos enfermeiros na transição do cuidado do hospital para o domicílio a partir de evidências na literatura. Trata-se de revisão integrativa, realizada em agosto de 2015, mediante busca nas bases de dados PubMed, Cumulative Index to Nursing and Allied Health Literature, Web of Science, SCOPUS e Literatura Latino-Americana e do Caribe em Ciências da Saúde. Foram selecionados 22 artigos que atenderam aos critérios de inclusão. Identificaram-se estudos de intervenção, pesquisas experimentais, quaseexperimentais, em idosos e com doenças crônicas. Os resultados evidenciaram cinco categorias temáticas com as principais atividades dos enfermeiros na transição do cuidado: planejamento de cuidados para a alta; auxílio na reabilitação social; educação em saúde; articulação com os demais serviços; acompanhamento pós-alta. Conclui-se que há necessidade de aprimoramento das práticas assistenciais e organização das atividades dos enfermeiros, promovendo coordenação do cuidado com foco na transição do hospital para o domicílio. Objective: This paper provides a theoretical-refl ective study of knowledge translation concepts and their implementation processes for using research evidence in policy and practice. Results:The process of translating research into practice is iterative and dynamic, with fl uid boundaries between knowledge creation and action development. Knowledge translation focuses on co-creating knowledge with stakeholders and sharing that knowledge to ensure uptake of relevant research to facilitate informed decisions and changes in policy, practice, and health services delivery. In Brazil, many challenges exist in implementing knowledge translation: lack of awareness, lack of partnerships between researchers and knowledge-users, and low research budgets. Conclusions: An emphasis on knowledge translation has the potential to positively impact health outcomes. Future research in Brazil is needed to study approaches to improve the uptake of research results in the Brazilian context. Keywords: Information dissemination. Evidence-based practice. No Brasil, existem muitos desafi os para implementar a translação do conhecimento: falta de familiaridade; falta de parceria entre pesquisadores e usuários do conhecimento, e baixos orçamentos para pesquisa. Conclusões: Uma ênfase na translação do conhecimento tem potencial para impactar positivamente resultados de saúde. Objective: To evaluate the quality of care transition for patients with chronic diseases and to verify its association with hospital readmission within 30 days after discharge. Method: Cross-sectional epidemiological study of 210 patients with chronic diseases discharged from a hospital in southern Brazil. The Care Transition Measure-15 (CTM-15) instrument was used, through a telephone contact and, in order to identify readmissions within 30 days, the hospital management system was consulted. Student’s t-tests analysis of variance and nonparametric Pearson or Spearman correlation tests were performed. Results: CTM-15 score was 74.7 (± 17.1). No significant association was found between the quality of care transition and hospital readmission. 12.3 % of the patients were readmitted, and 46.2 % of these readmissions were to the emergency service. Conclusions: The quality of the care transition for chronic patients from inpatient units to home, showed a satisfactory score. However, there was no association between the quality of care transition and hospital readmission within 30 days after discharge.
Objective: To evaluate the quality of the care transition for patients with chronic non-communicable diseases discharged from the emergency department to home. Method: A cross-sectional observational and epidemiological study conducted at an emergency department in the South of Brazil with 117 patients and 81 caregivers. The Care Transitions Measure was applied by phone to collect data. A descriptive and analytical statistical analysis was performed. Results: The quality of the care transition’s total score was close to satisfactory (69.5). The “Self-Management Training” factor had the highest score (70.6), while “Understanding medications” had the lowest (68.3). Items related to understanding medications and confidence in carrying out care after discharge obtained lower scores. Conclusions: A moderate quality of the care transition was evidenced, as well as the need to adopt strategies to improve the emergency department discharge process and the continuity of the care of patients with chronic diseases.
