Objective: to evaluate the quality of the sputum sample before and after the Nursing guidance to patients. Methods: this is a quasi-experimental research design, single group type, before and after, non-randomized study. The study enrolled patients with suspected pulmonary tuberculosis, respiratory symptomatic patients for over three weeks, aged over 18 years, of both genders and without tuberculosis history in the last two years. The educational intervention consisted of individualized guidance on the collection of sputum sample, which was based on the guidelines of the Ministry of Health of Brazil and on the explanatory folder delivery. Results: in this study participated 138 patients with suspected pulmonary tuberculosis. The results showed significant increase of the samples with purulent particles, volume greater than 5 mL and increased rate of patients diagnosed with tuberculosis, after the educational intervention. Conclusion: it was shown that after the educational intervention, it was observed sputum samples with better quality, with satisfactory aspect and volume for the effectiveness of the bacilloscopic examination.

RESUMO Objetivo: investigar como se desenvolveu a transferência da política do Tratamento Diretamente Observado da Tuberculose na ótica/para dos/os profissionais de saúde comunitária do Grupo Hospitalar Conceição. Método: trata-se de uma pesquisa de abordagem quantitativa do tipo inquérito epidemiológico, que se baseou na aplicação de um instrumento validado e autodirigido para os profissionais de saúde pertencentes às 12 Unidades de Atenção Primária à Saúde do Serviço de Saúde Comunitária do Grupo Hospitalar Conceição. Para a análise,foram utilizados a beta regressão, a descrição das frequências das variáveis, a distribuição da média, mediana e o desvio-padrão. Além disso, foram criados a média padronizada e os escores padronizados. Resultados: participaram do estudo 109 profissionais de saúde, a maioria do sexo feminino (77,06%), com idade entre 30 e 59 anos (87,16%) e pertencentes à categoria médica (37,61%). Em relação às três dimensões analisadas do processo de transferência da política, observou-se que a dimensão Conhecimento foi a que obteve maior escore (77,8%), sendo seguida pelas dimensões Informação (72,7%) e Inovação (67,7%). A necessidade de envolvimento e participação da população nas discussões do Tratamento Diretamente Observado, de melhoria da infraestrutura dos serviços de saúde e a complexificação do processo de realização dessa prática foram alguns, dentre outros, aspectos importantes apontados. Conclusão: urgem medidas de reformulações e melhorias nas ações vinculadas à operacionalização e transferência do Tratamento Diretamente Observadoda Tuberculose.
Objective: To analyze whether user-centered nursing practices in TB DOT are being carried out and are contributing to democracy. Method: A cross-sectional, exploratory, descriptive study with 123 nurses in three capital cities with high incidence of TB (Manaus-AM, João Pessoa-PB, Porto Alegre-RS) in Brazil. Data were collected from January to June 2014. Descriptive and multiple correspondence analyzes were used. Results: It was identified that the most user-centered care actions had scores below 50%, considered unfavorable to the exercise of democracy. In the analysis of the general user-centered practice score per municipality, Manaus and João Pessoa presented unfavorable results, and the municipality of Porto Alegre was partially favorable. Conclusion: The three municipalities need to advance in the implementation of user-centered practices in DOT, because their health system still has difficulties and challenges to overcome.
