Objectives:To delineate the clinical features and outcomes of dengue infection and to guide clinician of early diagnosis and identification of risks factors for dengue hemorrhagic fever.Methods:This study is a retrospective cross-sectional. Clinical records of 567 patients with a confirmed diagnosis of dengue infection, admitted to a single hospital in Jeddah, Saudi Arabia, between January 2010 and June 2014 were reviewed.Results:Dengue infection was most common in adult males. Sixty-eight percent of infections were in Saudi nationals. In addition to the diagnostic clinical features, leucopenia and thrombocytopenia were typical of dengue infection. Approximately 4.1% of adult patients and 7.1% of pediatric patients developed dengue hemorrhagic fever (DHF). Abdominal pain and vomiting were more common in patients developing DHF. Mean platelet count was lower in adult, but not pediatric patients developing DHF. Peak alanine aminotransferase (ALT) was higher in adult and pediatric patients developing DHF. Three patients died, 2 of them developed DHF. Ninety-eight percent of adult patients and 92% of pediatric patients made a full recovery.Conclusions:Dengue infection is common in Jeddah. Abdominal pain and vomiting, thrombocytopenia, and elevated ALT are typical of severe infection, which is more likely to be associated with significant morbidity and mortality.
Background: Laparoscopic sleeve gastrectomy (LSG) is the most commonly performed surgery to treat morbid obesity. Post-LSG leak and stenosis are serious complications that can be associated with significant morbidity and mortality. Objective: The objective was to report the efficacy and safety profile of using specifically designed fully covered self-expandable metallic stent for the treatment of post-LSG complications. Methods: This retrospective study included adult patients who underwent placement of a fully covered esophagogastric, self-expandable metallic stent for post-LSG leak or stenosis. The procedure was carried out at King Abdulaziz University Hospital, Jeddah, Saudi Arabia, between September 2017 and May 2019. Data regarding demographics, indication for stenting, size of the stent, procedural success and poststenting adverse events were collected. Results: A total of 14 patients met the inclusion criteria, with indication for endoscopic stenting being post-LSG leak in 11 patients and stenosis in 3 patients. The technical success rate of self-expandable metallic stent placement was 100%, and the clinical success was 85.7% (12 of 14 patients). Nausea (71.4%) and vomiting (85.7%) were the most frequent mild adverse events reported. Stent-induced esophageal stricture was the only major adverse event reported in two patients. Conclusion: Placement of specifically designed self-expandable metallic stent for the treatment of post-LSG leak and stenosis is an effective and safe approach. Further studies with larger cohorts are needed to assess the optimal duration needed to treat such complications.
AbstractsConclusions 88 transfers for gastroschisis were conducted over the period. 91% babies were kept within region and transferred within an average time of 80 min in keeping with the network guideline. There have been no patient related clinical incidents. Purpose Antenatally detected urinary tract abnormalities (ADUTA) are increasingly recognized. Our aims were to determine the incidence and outcomes of antenatally diagnosed congenital hydronephrosis in a large cohort. Methods We reviewed the records of 18,853 deliveries between January 2008 and December 2011 at King Abdulaziz University Hospital, Saudi Arabia. ADUTA were recorded and their postnatal medical records were reviewed for demographic and radiological data. Results ADUTA were diagnosed in 327 fetuses (1.7%). The commonest pathology was congenital hydronephrosis (n=313, 95.7%). Cystic renal anomalies were reported in four babies (1.2%) and 10 children (3.1%) were reported to have other renal anomalies, including duplex kidneys or a single kidney. Two-hundred and forty babies with congenital hydronephrosis were followed-up. Hydronephrosis resolved in 99 children (41.2%) within 2 months of birth. Twentynine subjects had underlying renal anomalies (12.1%), including vesico-uretral reflux (n=12, 5%), pelvi-uretric junction obstruction (n=14, 5.8%) and posterior urethral valve (n=3, 1.25%). The best predictor for non-resolving congenital hydronephrosis and underlying anatomical abnormalities was the AP diameter on the first postnatal scan. A cut-off point of 5 mm was found to be 83% sensitive in predicting non-resolving hydronephrosis, while 7 mm was 88% sensitive and 10 mm was 94% sensitive. Conclusion Congenital hydronephrosis is the commonest ADUTA. A large percentage resolved within 2 months of birth, but underlying anatomical abnormalities were found in 12.1%. All babies with antenatally detected hydronephrosis should be examined by US postnatally but further radiological investigations should only be performed for persistent significant AP dilatation ≥10 mm.
