The prevalence of COPD in non-smoking individuals from rural and urban Vietnam and Indonesia was 6.9%, of which a significant proportion (94%) were previously undiagnosed.
Background Although stem cell transplantation has been successfully performed for cerebral palsy (CP) related to oxygen deprivation, clinical trials involving the use of stem cell transplantation for CP related to neonatal icterus have not been reported. The aim of this study was to evaluate the effectiveness of transplantation of autologous bone marrow mononuclear cell (BMMC) for improving gross motor function and muscle tone in children with CP related to neonatal icterus. Methods This open-label, uncontrolled clinical trial, which included 25 patients with CP related to neonatal icterus who had a Gross Motor Function Classification System (GMFCS) score between level II and level V, was conducted between July 2014 and July 2017 at Vinmec International Hospital (Vietnam). BMMC were harvested from the patients’ iliac crests. Two procedures involving BMMC transplantation via the intrathecal route were performed: the first transplantation was performed at baseline, and the second transplantation was performed 6 months after the first transplantation. Gross motor function and muscle tone were measured at three time points (baseline, 6 months, and 12 months) using the Gross Motor Function Measure (GMFM) and the Modified Ashworth Scale. Results In this trial, we observed significant improvement in gross motor function and a significant decrease in muscle tone values. Total score on the 88-item GMFM (GMFM-88), scores on each GMFM-88 domain, and the 66-item GMFM (GMFM-66) percentile were significantly enhanced at 6 months and 12 months after the first transplantation compared with the corresponding baseline measurements ( p -values < 0.05). In addition, a significant reduction was observed in muscle tone score after the transplantations ( p -value < 0.05). Conclusion Autologous BMMC transplantation can improve gross motor function and muscle tone in children with CP related to neonatal icterus. Patient satisfaction has been considered an important metric to measure and improve the quality of health care services. Aims: This study aimed to capture patient satisfaction of the quality of services and explore the disparities in patients’ satisfaction by different type of health facilities. Methods: A cross-sectional investigation was conducted in 886 participants following the guidelines of Ministry of Health on assessment of patients’ satisfaction of out-patient services with 5 main indices: the average patient satisfaction score (PSI), the average patient satisfaction percentage, comprehensive satisfaction index (CSI), percentage of continuing to utilize health services, and percentage of perceiving health care expectaion. Results: The PSI of outpatient services among all partcipants reached 3.75 out of 5 points, corresponding to a satisfaction percentage of 64.8%. Of which, the PSI on different attributes of healthcare settings varied from 3.69/5 (lowest for district-level health facilities) to 3.92/5 (highest for private health settings). CSI was at 13.5%, the percentage of continuing to utilize health services reached 96.2% and people perceived 81.5% of their expectation. Primary care facilities have lower levels of satisfaction than the provincial, central and private facilities (p<0.05). Conclusion: Robust efforts should be taken in investing the infrastructure and performance of primary care settings, and improving the administrative procedures of public health settings. Furthermore, local authority should pay attention to promote the awareness of community to availability of health services at primary care settings.
Patient-centered care is widely acknowledged as an important goal in healthcare delivery. Research has demonstrated that patient perceptions of patient-centred care can be linked to clinical and long-term outcomes in addition to being a useful metric for quality improvement efforts. Objectives: 1) To identify patients’ perspectives and experiences of patient-centerd care in some communes in Thua Thien Hue province; 2) To determine the association between the patients’ perspectives, experiences on patient-centered care and utilization of the health services. Methods: A cross sectional descriptive study was conducted in 313 patients living in Thua Thien Hue province. A structured questionnaire was use to investigate 4 main domains of patient-centered care: Information and Education, Relationships with doctors and other health professionals, making decisions, motivate patients about self-management. We used test ANOVA to analyze the association between patient-centered care and health care utilization among participants (p < 0.05). Results: The percentage of good patient-centered care (≥ 3.75 points) was 43.1% (Mean = 3.58 (0.57)). Patients using health services at the commune health centers had a higher experience in patient-centered care compared to district, province and central hospitals (p < 0.005). Domains “Encourage patients to take care of themselves” and “Relationship factor with doctors” were evaluated low. There were statistically significant associations between patient-centered care and primary health care facilities, quality of life utilization of periodic health checkup, and patient activation in health care (p <0.05). Conclusion: Out study illustrates that patients’ perspectives and experiences of patient-centered care are still low.
