<p class="abstract">Livedoid vasculopathy (LV) also known as livedoid vasculitis is a rare and chronic thrombotic vasculopathy that typically affects the lower extremities. Diagnosis of LV are not well defined and the treatment are widely varied. We report a case of 30 years old female patient diagnosed with Livedoid vasculopathy and systemic lupus erythematous as underlying diseases. Patient presented with chief complain painful ulcer on her bilateral lower extremities. Hypercoagulable state was normal. Skin biopsy found that dermal vessels filled with erythrocytes extravasation and inflammation cell PMN. ANA profile was positive for RNP/Sm suggest that a systemic lupus erythematosus. Patient underwent successful treatment planning for methylprednisolone 16 mg twice daily, hydroxychloroquine 200 mg once daily and methotrexate 10 mg once a week. The ulcer completely resolved after 2 months of the treatment and had no recurrent ulcer. LV is a rare, chronic, and occlusive disease of the veins supplying the upper parts of the skin. Histopathological finding is thickening or hyaline changes in the walls of superficial dermal vessels and luminal fibrin deposition. Red cell extravasation and perivascular lymphocytic infiltrates are expected findings. SLE was underlying condition that cause endothelial injury and leads to thrombosis of the dermal vessels. no treatment guidelines are available for LV. Methylprednisolone, hydroxychloroquine and methotrexate have successful therapeutic response to a patient with ulcerative LV and SLE as underlying diseases.</p>
Dermatitis atopik (DA) merupakan masalah yang sering muncul pada negara berkembang dan sedang berkembang. DA merupakan penyakit inflamasi kronik pada kulit yang terjadi pada 15% sampai 25% pada anak dan 3% pada dewasa. Penyakit ini menyebabkan morbiditas dan memiliki efek pada kualitas hidup. Tujuan utama dilakukann penelitian ini adalah untuk mengetahui efek vitamin D pada derajat keparahan DA pada anak. Penelitian dilakukan di poliklinik kesehatan kulit dan kelamin rumah sakit umum daerah dr. Zainoel Abidin Banda Aceh selama 4 bulan dimulai sejak 15 Mei hingga 15 september 2018. Penelitian ini merupakan penelitian uji klinis desain paralel. Sebanyak 68 pederita DA terlibat dalam penelitian ini dan dibagi menjadi dua kelompok yaitu kelompok yang mendapat vitamin D 600 IU selama 28 hari dan kelompok tanpa suplementasi vitamin D dengan jumlah masing – masing kelompok 34 orang. Seluruh pasien yang terlibat mendapatkan terapi standar berupa steroid topikal dan antihistamin secara oral. Tingkat keparahan gejala klinis sebelum dan setelah pengobatan diukur menggunakan Scoring Dermaitis Atopic (SCORAD). Data hasil penelitian dianalisis menggunakan analisis statistik tidak berpasangan dengan tingkat kepercayaan 95%. SCORAD penderita DA yang mendapat suplementasi vitamin D selama 28 hari dengan terapi standar (13,8 ± 6,39) lebih baik dibandingkan dengan kelompok yang tidak mendapat vitamin D 20,6 ± 7,94 dengan tingkat kemaknaan p < 0,001. Vitamin D merupakan prohormon yang memiliki fungsi utama mengatur keseimbangan kalsium tubuh. Sebagai imunoregulasi, vitamin D memiliki dua fungsi penting sebagai hormon sekosteroid pada regulasi hemostasis kalsium pada tubuh dan sebagai zat esensial organik yang sangat penting terhadap respons imun. Suplementasi vitamin D selama 28 hari lebih efektif menurunkan derajat keparahan DA pada anak dibandingkan terapi standar.
Scabies is a parasitic skin disease caused by Sarcoptes scabiei var. hominis. Crusted scabies, also known as Norwegian scabies, is a rare and severe form of scabies that occurs more frequently in immunocompromised, elderly, disabled, and debilitated patients. The present study reports a case of crusted scabies combined with bullous scabies. A 44-year-old man was admitted with the chief complaint of red, thick, scaly patches of skin accompanied by crusting that felt itchy and worsened at night. The lesions had appeared all over his body intermittently for the last 10 years; however, the complaints worsened in the last 2 months. From dermatological status, the anterior and posterior thoracic regions and between the fingers of both hands showed generalized distribution of erythematous, yellowish, hyperkeratotic plaques with clear boundaries, pustules, and blisters on an erythematous base with multiple erosions and excoriations. S. scabiei were found on skin scraping examination, and histopathological examination supported the diagnosis of bullous scabies. The patient received 5% permethrin lotion topical therapy and systemic ivermectin therapy. In addition, the patient was given systemic antibiotics. The lesions improved after 2 weeks of therapy, and no S. scabiei were found on the subsequent microscopic examination. Treatment with a combination of topical and systemic scabicidal agents produced a favorable outcome in this patient.
Background: Infantile hemangiomas (IH) are the most prevalent benign tumors in children, predominantly in girls. Most IH cases do not require treatment due to spontaneous involution, but 10% of cases do require early treatment due to size, location, and complications. Purpose: This study tried to define the profile, clinical aspects, and therapy of IH in Dr. Zainoel Abidin General Hospital, Banda Aceh. Methods: A descriptive approach using a retrospective analysis employing secondary data from the outpatient clinic's patient registration book. This study included all new cases of infantile hemangioma at the outpatient clinic general hospital between 2013 and 2021. Result: A total of 105 patients were involved in this study. The majority of IH patients (59%) were under the age of one year, with 64 female patients (61%) making the most visits to the pediatric clinic in 2017, 2018, and 2019 (77.1%). The most prevalent clinical features of skin lesions were nodules in 69 individuals (65.7%), and the most common site was the facial region (13.3%) in 44 patients. In 20 individuals, a combination of propanolol and oral corticosteroids was used as the most effective treatment strategy (19% ). Conclusion: Infantile hemangiomas is the most frequently reported benign tumor in children. Females are more likely than males to develop IH. The majority of IH is minor and normally resolves on its own without therapy. However, certain rare kinds of IH require clinician attention and systematic treatment.
Vitiligo menimbulkan perbedaan jelas antara kulit depigmentasi dan kulit normal yang menyebabkan pasien vitiligo sering menghadapi stigmatisasi dan masalah psikososial sehingga rentan terhadap depresi, kecemasan, dan stres. Tujuan penelitian ini adalah untuk mengetahui hubungan antara derajat keparahan vitiligo dengan tingkat depresi pasien vitiligo. Penelitian ini merupakan penelitian analitik observasional dengan desain cross-sectional. Pengumpulan data menggunakan teknik consecutive sampling dan pengisian kuesioner Back Depression Scale pada Juli hingga Oktober 2022 di Poliklinik Kulit dan Kelamin Rumah Sakit Umum Daerah dr.Zainoel Abidin Banda Aceh. Kriteria inklusi meliputi pasien dengan diagnosis vitiligo, usia 13-80 tahun, dan bersedia mengikuti penelitian; sedangkan kriteria eksklusi adalah pasien vitiligo yang telah didiagnosa dengan depresi, dan telah atau dalam proses pengobatan depresi. Didapatkan hasil dari 31 sampel sebanyak 48,4% laki-laki, 51,6% perempuan, usia rata-rata 36 tahun, 32,4% merupakan siswa dan mahasiswa, guru/dosen dan PNS masing-masing 12,9%. Jenis klinis vitiligo localized sebesar 58,1 %, vulgaris 29%, dan acrofacial 12,9%. Sebanyak 74,2% telah mengalami vitiligo selama 1-5 tahun, 54,6% telah mendapatkan pengobatan selama 1-5 tahun; 64,5% dengan riwayat pengobatan topikal dan fototerapi. Mayoritas (93,5%) pasien vitiligo tidak memiliki riwayat autoimun lainnya, 87,1% memiliki tingkat keparahan vitiligo ringan, 6,5% vitiligo sedang, dan 6,5% vitiligo berat. Sebagian besar pasien (93,5%) tidak mengalami depresi, dan 6,5% menderita depresi ringan. Berdasarkan analisa bivariat didapatkan hubungan yang lemah antara derajat keparahan vitiligo dengan tingkat depresi (p= 0.66), sehingga disimpulkan bahwa tidak terdapat hubungan antara derajat keparahan vitiligo dengan terjadinya depresi.
