No período de cinco anos, 87 doentes com trauma pancreático foram tratados por pancreatectomia corpo-caudal em cinco serviços de emergência. A idade variou de 7 a 64 anos (média de 28), sendo que 73 (84%) eram do sexo masculino. O principal mecanismo de trauma foi o ferimento penetrante em 72,5%, com ferimentos por projéteis de arma de fogo em 3/4 dos casos. Lesões associadas ocorreram em 96% dos doentes. A extensão da ressecção pancreática correspondeu de 30% a 70%, em 62% dos casos. O baço foi preservado em 45,6% dos 57 doentes que se apresentaram sem lesão esplênica. A taxa de complicações foi de 40,2%, sendo a fístula pancreática e a pancreatite pós-operatória as mais freqüentes. A mortalidade total foi de 12 doentes (13,7%). As conclusões deste estudo foram as seguintes: a) há relação nas taxas de morbidade com o mecanismo de lesão; b) a morbidade é mais freqüente em doentes com trauma fechado; c) não há diferença na incidência de complicações com o método de fechamento do pâncreas remanescente nem com a extensão da ressecção; d) não há relação da mortalidade com o mecanismo de trauma; e) há uma tendência no aumento da taxa de mortalidade no trauma fechado com presença de lesões associadas.
OBJECTIVE:To evaluate the effect of pneumoperitoneum, both alone and in combination with controlled ventilation, on peritoneal lymphatic bacterial clearance using a rat bacterial peritonitis model.METHOD:A total of 69 male Wistar rats were intraperitoneally inoculated with an Escherichia coli solution (109 colony-forming units (cfu)/mL) and divided into three groups of 23 animals each: A (control group), B (pneumoperitoneum under 5 mmHg of constant pressure), and C (endotracheal intubation, controlled ventilation, and pneumoperitoneum as in Group B). The animals were sacrificed after 30 min under these conditions, and blood, mediastinal ganglia, lungs, peritoneum, liver, and spleen cultures were performed.RESULTS:Statistical analyses comparing the number of cfu/sample in each of the cultures showed that no differences existed between the three groups.CONCLUSION:Based on our results, we concluded that pneumoperitoneum, either alone or in association with mechanical ventilation, did not modify the bacterial clearance through the diaphragmatic lymphatic system of the peritoneal cavity.
Twenty-six patients showing peritonitis due to nontraumatic acute abdomen were submitted to ostomy. Mean age was 51 years (range 25-83), being 13 males and 13 females. Bowel obstruction (BO) was the most frequent cause of peritonitis (11 cases), followed by intestinal perforation (IP) (8 cases), acute mesenteric infarction (AMI) (5 cases), and acute abdomen of inflammatory/infectious origin (AAIO) (2 cases). Brook's ileostomy was performed on 65% of the patients. Jejunostomy was performed only in 4 patients, leading to a bad evolution. Overall mortality was 54%. Primary ostomy or anastomosis in cases of peritonitis constitute a highly controversial theme. Indications and problems involving the intestinal exteriorization in emergency surgery urgency are herein discussed.
