Considering the growing number of requests from the criminal investigations authorities addressed to the institutions of legal medicine, testing of blood alcohol concentration both in the living person and in the corpse, we believe that a presentation of the two methods which are used in our country, is a topic of interest at present. The purpose of this article is to provide the reader with the technical details on how blodd alcohol concentration is realised by means of the gas chromatographic method and the classical one, (Cordebard modified by D. Banciu and I. Droc) respectively. Another purpose of this article is to also show, in a comparative way, the elements that make the gas chromatographic method superior to the former one.
Ethylene glycol intoxication is potentially fatal and associated with typical clinical, laboratory and histopathological findings. The authors present the case of a 57-year-old male with a history of chronic alcoholism and who accidentally ingested approximately 1 litter of antifreeze solution. The patient was discovered comatose in his house and addressed to the emergency department with a Glasgow coma score of 3, severe metabolic acidosis, acute renal failure, atrial fibrillation and liver dysfunction. Despite reanimation manoeuvres and haemodialysis for 2 h the patient deceased 5 h after hospital admission. Necropsy examination revealed a stomach with oedematous walls, mucosa erosions and signs of bleeding together with a disorganised, granular single kidney with unidentifiable corticomedullary border. Histopathological examination displayed typical findings in the kidney such as autolytic changes of the epithelium and abundant calcium oxalate crystals in the lumen of the proximal tubules. Ethylene glycol intoxication is frequent in our country and its metabolites glycoaldehyde, glycolic acid, glyoxylic acid and oxalic acid are responsible for the severe metabolic acidosis and formation of calcium oxalate crystals in various organs and leading to severe multiple organ dysfunction and death. Forensic pathologists should be aware of clinical and biological manifestations as well as of typical histopathological findings as ethylene glycol is commonly ingested accidentally or used in homicidal/autolytical attempts.
Stereotyping is a phenomenon often met in society and it is manifested by stigmatization and discrimination. This phenomenon gains a deeply negative character when it is manifested in providing healthcare to some groups of people, races, and ethnicities. This paper is based on data obtained during a qualitative research aimed to correlate the necessity of the end-of-life care of a Rroma patient with Rroma traditions and the existing politics and services in the Romanian healthcare system. Our research was based on 48 interviews, conducted on chronic ill persons and caregivers, in Rroma communities from two regions of Romania (Cluj and Iaşi). The aim of forensic autopsy is to accurately reconstitute, as much as possible, the circumstances of death (the cause of death and the mechanism of death, the post-mortem interval, the related pathology and its role, the survival interval). Post-mortem biochemical analyses can help to various extents in establishing these issues. In order to elucidate the issue of post-mortem biochemical investigations, we conducted an analysis of forensic casuistry within a delimited territory, for a period of 3 years in order to identify the elements necessary for the diagnosis of hypertonic dehydration. By grouping the cases depending on the immediate cause of death, the diagnosis criteria for hypertonic dehydration showed the possibility of performing the differential diagnosis by biochemical analyses performed in vitreous humor (sodium, chlorine, urea, creatinine), the criteria being not met in any of the other groups.
