The purpose of this study was to translate and integrate nursing diagnosis concepts from the Clinical Care Classification (CCC) System Version 2.0 to DiagnosticPhenomenon or nursing diagnostic statements in the International Classification for Nursing Practice (ICNP) Version 1.0. Source concepts for CCC were mapped by the project team, where possible, to pre-coordinated ICNP terms. The manual decomposition of source concepts according to the ICNP 7-Axis Model served to validate the mappings. A total of 62% of the CCC Nursing Diagnoses were a pre-coordinated match to an ICNP concept, 35% were a post-coordinated match and only 3% had no match. During the mapping process, missing CCC concepts were submitted to the ICNP Programme, with a recommendation for inclusion in future releases.
Objective: To propose approaches to improve nursing handoffs for surgical patients, including standardization. Background: Handoffs, or the transfer of accountability and patient information, can generate potential risks for patient safety. Standardization has been proposed to help improve handoffs. Methods: After observing 333 nursing handoffs in the surgical wards of our institution, we conducted a thematic content analysis, comparing and contrasting the observations. Results: Handoff processes, including the use of support tools, varied among the observations. Common themes in the handoff content suggested possibilities of standardization. About half of the 51 interruptions occurring during the observed handoffs were by healthcare professionals. Conclusions: Standardization to improve handoffs should address both the content and the process. Interruptions were common and should be avoided whenever possible. Future studies should also consider the use of mobile applications to support handoffs and clinical documentation.
Lors de la prise en charge du patient en situation de fin de vie, l’accompagnement des proches aidants implique que l’équipe soignante soit préparée à prodiguer des soins qui nécessitent de l’écoute, de l’empathie et de la disponibilité. Sur la base d’une enquête transversale explorant les besoins de 103 professionnels de santé, une formation comprenant trois ateliers, sur les thèmes de l’accompagnement des proches aidants, des soins palliatifs et de la fin de vie, a été proposée. Au total, 131 soignants de cinq unités y ont participé. Au terme des ateliers, un sondage a été réalisé auprès des participants afin d’identifier les pistes d’amélioration à mettre en place.
