Public Health Center is defined as a health service facility which serves the efforts of public health and firstlevel individual health, emphasizing the promotive and preventive efforts. There are obstacles and problems encountered by the health sector in which they need to be coped with properly. The problems faced by Public Health Center on registrasion process, medical examination, the pharmacy until the manual-reporting and the patient who forgot to bring the medical index card (KIB). Therefore, an information system of the outpatient service by using fingerprint technology was needed to increase the time-efficienc and help the staff in identifying the old. The method used in this research was waterfall. Concerning the process of designing the system, Flowchart system, Context Diagram, Data Flow Diagram, Entity Relationship Diagram were used; in addition, Microsoft visual audio version 1.46 was performed in implementing the program. The result of this research revealed that there was an information system of outpatient service by using fingerprint technology as a substitute for a medical index card. The strength of an information system designed by the researcher utilized the fingerprint technology to identify the patient and officer's login in which it kept them from forgetting their username and password.
