Aortitis is one of the many possibilities of lesion caused by tertiary syphilis. Between all the cardiovascular lesions, the aorta's injury is the most common. We report a case of a 48-year-old patient diagnosed with syphilitic aortitis who had undergone surgery for the replacement of the aortic root and aortic valve. The diagnosis hypothesis was pondered because of the in situ aspect of the arterial damage. Although the rarity of the disease, it persists.

Fístulas arteriovenosas coronárias pertencem a um grupo de lesões cardíacas de difícil diagnóstico, por não apresentarem sintomas; quando suspeitadas de ocasionar o quadro clínico de um paciente, ou descobertas em exame de rotina, devem ser pesquisadas para o tratamento adequado. É relatado o caso de homem de 65 anos de idade, com história de precordialgia súbita, evoluindo para IAM de parede inferior, cuja investigação revelou a presença de fístulas de artérias coronárias direita e esquerda para tronco pulmonar. Após tratamento clínico seguido da operação, demonstrou resultados satisfatórios.
Coronary arteriovenous fistulas belong to a group of cardiac pathologies that are rarely diagnosed because they do not present symptoms. However, when suspected of causing the clinical picture, they must be investigated to determine adequate treatment. The case of a 65-year-old man with a history of sudden angina, progressing to acute myocardial infarction of the inferior wall is presented. After diagnostic investigation, the presence of right and left coronary arteriovenous fistulas to pulmonary artery were found. After surgical and clinical treatment, the patient's course was satisfactory
