AbstrakTekanan parsial oksigen darah arteri (PaO2) sebagai penanda oksigenasi dalam darah arteri dapat diukur dari analisis gas darah. Hipoksemia ditandai dengan PaO2<80mmHg. Hipoksia menimbulkan peningkatan jumlah Eritrosit Berinti (EB) sebagai kompensasi kebutuhan oksigen yang meningkat. Tujuan penelitian ini adalah menentukan korelasi PaO2 dengan jumlah EB pada neonates hipoksemia di RSUP Dr M Djamil Padang. Penelitian analitik dengan rancangan potong lintang dilakukan terhadap neonatus yang dirawat di RSUP Dr M Djamil Padang dengan PaO2 < 80mmHg. Penelitian dilaksanakan Februari sampai Agustus 2016. Pemeriksaan PaO2 dilakukan melalui analisis gas darah dan EB dihitung dengan melihat sediaan hapus darah tepi melalui mikroskop. Hasil pemeriksaan dianalisis menggunakan korelasi Spearman dan bermakna bila p<0,05. Hasil penelitian terhadap 30 sampel didapatkan rerata PaO2 (mmHg) pada hipoksemia ringan dan hipoksemia sedang-berat berturut-turut 71,40(5,36), 48,67(12,53) dan rerata EB/100leukosit 11,53(10,43), 26,87(15,25). Berdasarkan analisis korelasi Spearman didapatkan nilai r: -0,257 pada hipoksemia ringan. Nilai r: -0,280 pada hipoksemia sedang-berat. Korelasi PaO2 dengan EB berkorelasi lemah dan tidak bermakna secara statistik. Hasil PaO2 dengan EB berkorelasi lemah dan tidak bermakna secara statistik.Penelitian lebih lanjut dengan tambahan parameter lain seperti pH, pCO2 dan menghubungkan lama hipoksemia atau hipoksia dengan jumlah EB pada neonatus. The number of nucleated red blood cells (nRBCs) increases to compensate hypoxia. The objective of this study was to assess correlation between PaO2 and nRBCs in hypoxemia neonates. This cross-sectional analytic studies was conducted in Dr M Djamil Hospital to inpatients subject with PaO2 < 80mmHg from February-August 2016.Level of PaO2 was analyzed by blood gas measured and nRBCs were determined by blood smears. Spearman's test was used to analyze statistic and considered significant if p<0.05. Mean of PaO2 (mmHg) 71.40(5.36), 48.67(12.53), and nRBc 11.53 and 26.87 per 100 white blood cell (WBC) for mild hypoxemia and moderat hypoxemia respectively.Spearman's correlation test showed r: -0.257 for mild hypoxemia, r:-0.280 for moderate hypoxemia. There is weakly correlation between PaO2 and nRBCs and statistically not significant. There is weakly correlation between PaO2 and nRBCs and statistically not significant. Future studies are needed to assess correlation between pH, pCO2, longterm hypoxemia/hypoxia and nRBCs
Abstrak Diagnosis Myelodysplastic syndrome (MDS) ditegakkan berdasarkan pemeriksaan hematologi, morfologi sel darah tepi, sumsum tulang, pemeriksaan lanjutan sitogenetika dan imunofenotiping, namun dua pemeriksaan terakhir ini tidak tersedia di semua rumah sakit di Indonesia sehingga diagnosis MDS terbatas pada pemeriksaan morfologi sel darah tepi dan sumsum tulang. Tujuan: Mengetahui gambaran hematologi pasien MDS di RSUP Dr. M. Djamil Padang. Ini merupakan penelitian deskriptif di laboratorium sentral RSUP Dr. M. Djamil Padang dari bulan November 2016 sampai dengan Oktober 2017. Populasi adalah semua pasien yang melakukan pemeriksaan sumsum tulang. Sampel penelitian adalah semua pasien yang telah didiagnosis MDS berdasarkan hasil pemeriksaan hematologi, sediaan hapus darah tepi dan sumsum tulang. Hasil: Didapatkan 19 penderita MDS. Pemeriksaan: anemia 68,4%, anemia+organomegali 31,6%. Pemeriksaan hematologi: anemia+leukopenia+trombositopenia 57,8%, anemia+leukopenia 21,1%, anemia+trombositopenia 21,1%. Pemeriksaan sediaan hapus darah tepi: gambaran eritosit, ukuran: anisopoikilositosis 73,6%, normositik 26,4%; warna: polikrom 57,8%, normokrom 42,2%, ditemukan blast pada 42,1% penderita. Pemeriksaan sediaan hapus sumsum tulang, selularitas: hiposelular 73,6%, hiperselular 21,1%, normoselular 5,3%. Displasia seri eritropoietik+granulopoietik+trombopoietik 47,4%; seri eritropoietik+ granulopoietik 31,5%; seri eritropoietik+trombopoietik 15,8%; seri eritropoietik saja 5,3%. Simpulan: Gambaran hematologi pasien MDS terbanyak pada penelitian ini adalah anemia, anisopoikilositosis, polikrom, selularitas hiposelular, dan ditemukannya displasia seri eritropoietik, granulopoietik dan trombopoietik. Stored red blood cells undergo morphological and biochemical changes with increased storage time, commonly refferred as the storage lesion, reduced integrity of erythrocyte membrane, causing hemolysis and increased free plasma hemoglobin level. Nitric Oxide (NO) is an endogenous vasodilator with the major role in vascular and blood flow regulation. Accumulation of free hemoglobin (fHb) during storage RBC hemolysis is thought to be correlated with elevated NO consuming causes low NO level that significantly impair endothelial function. This study aim to was to analyze the correlation of free Hb and plasma nitric oxide level during PRC storage period in the blood bank. This research was an analytic study with a cross-sectional design to 14 unit PRC those were stored in Dr. M. Djamil Padang Hospital Blood Bank. The study was conducted from May 2016 to August 2017. The assay was performed for 28 days of storage with one week intervals. Free plasma hemoglobin and NO level were analyzed by cyanmethemoglobin and colorimetry method respectively. Spearman test was used to access the correlation between free Hb level and nitric oxide plasma with p<0.05 means significant. Most donors were male (85.7%) with age range were 33(9) years. Free Hb and nitric oxide level were significantly differenced in each week storage periods (p<0.05). Two parameters was significantly had moderate negative correlation during storage periods (r=-0.56; p=0.01).
Malaria masih merupakan masalah kesehatan di daerah tropis dan sub tropis terutama Asia dan Afrika dengan angka kesakitan dankematian yang tinggi. Parasit masuk ke dalam darah selain menimbulkan gejala klinis berupa demam, juga diduga memicu terjadinyaperubahan hematologi antara lain monositosis dan trombositopenia. Penelitian ini bertujuan mengetahui uji diagnostik tolok ukurhematologi di pasien terduga malaria. Penelitian uji diagnostik potong lintang ini dilakukan terhadap 60 orang pasien terduga malariayang memenuhi patokan kesertan dan nonkesertaan masa waktu Juli 2015 sampai Maret 2016 di Instalasi Laboratorium Sentral RSUP.Dr. M. Djamil Padang, Rumah Sakit Tingkat III Reksodiwiryo Padang, Puskesmas Barung Belantai Kabupaten Pesisir Selatan, RumahSakit Hanafie Kabupaten Bungo, Rumah Sakit Sultan Thaha Saifuddin dan Puskesmas Rimbo Bujang Kabupaten Tebo. Tolok ukur yangdiperiksa selain mikroskopis malaria adalah hitung monosit dan trombosit. Analisis statistik menggunakan piranti lunak dan Tabel2×2. Kepekaan dan kekhasan demam, bertempat tinggal atau ditemukan riwayat perjalanan di daerah endemis malaria serta hitungmonosit >8% dan hitung trombosit <150.000/mm3 dibandingkan pemeriksaan mikroskopis pada penelitian ini berturut-turut adalah81,6% dan 81,8%. Nilai duga positif, nilai duga negatif, rasio kemungkinan positif dan rasio kemungkinan negatif pada penelitian iniberturut-turut adalah 88,6%, 72%, 4,5 dan 0,2. Penelitian ini mendapatkan kepekaan dan kekhasan demam, bertempat tinggal atauditemukan riwayat perjalanan di daerah endemis malaria serta hitung monosit >8% dan hitung trombosit <150.000/mm3 yang tinggidibandingkan pemeriksaan mikroskopis di pasien malaria.
