Objective: To report the importance of associating the use of ultrasonic inserts and the surgical operating microscope in the treatment of a lower molar with calcification located at the entrance of one of the root canals. Case Report: A female patient reported chewing sensitivity in tooth 47, already endodontically accessed and diagnosed with acute apical periodontitis. In the first session, during the attempt to access the root canals, the mesiobuccal canal was not located, requiring the use of an operating microscope associated with dental ultrasound to continue treatment. After locating the calcified canal, electronic odontometry of the canals, preparation and intracanal medication with calcium hydroxide paste were performed. In the second session, the dressing was removed, the cone test was performed, and obturation was performed. The tooth was restored and preserved after 15 months, and is asymptomatic, in function and with radiographic evidence of bone repair. Conclusion: The use of resources such as ultrasound and microscopy to help locate canals in cases of calcification is an excellent strategy to improve treatment prognosis.
A dor pós-operatória é um dos principais problemas relacionados ao preparo e desobstrução dos canais radiculares. Tal fato relaciona-se aos debris que extruem para o periápice radicular. O presente trabalho tem por objetivo avaliar a quantidade de debris extruídos pelo forame apical durante o retratamento endodôntico utilizando os sistemas reciprocantes Reciproc® e Wave One® e técnica convencional com limas manuais e brocas de gates glidden. Foram selecionados 45 pré-molares inferiores, instrumentados, obturados e separados em três grupos experimentais: Grupo1 -Reciproc®, Grupo2 -.Wave One® e Grupo 3: Convencional. Grupo controle negativo sem espécimes foi constituído. As desobstruções foram realizadas e debris extruídos via forame foram coletados e pesados através de uma balança analítica de precisão. O grupo 2 foi aquele que apresentou menor extrusão de debris quando comparado aos demais grupos. Pode-se concluir que, apesar dos sistemas reciprocantes terem extruído menos que a técnica convencional, não houve diferença estatística entre eles.
