SummaryBackgroundThe spectrum of central nervous system (CNS) abnormalities described in association with congenital melanocytic naevi (CMN) includes congenital, acquired, melanotic and nonmelanotic pathology. Historically, symptomatic CNS abnormalities were considered to carry a poor prognosis, although studies from large centres have suggested a much wider variation in outcome.ObjectivesTo establish whether routine MRI of the CNS is a clinically relevant investigation in children with multiple CMN (more than one at birth), and to subclassify radiological abnormalities.MethodsOf 376 patients seen between 1991 and 2013, 289 fulfilled our criterion for a single screening CNS MRI, which since 2008 has been more than one CMN at birth, independent of size and site of the largest naevus. Cutaneous phenotyping and radiological variables were combined in a multiple regression model of long‐term outcome measures (abnormal neurodevelopment, seizures, requirement for neurosurgery).ResultsTwenty‐one per cent of children with multiple CMN had an abnormal MRI. Abnormal MRI was the most significant predictor of all outcome measures. Abnormalities were subclassified into group 1 ‘intraparenchymal melanosis alone’ (n = 28) and group 2 ‘all other pathology’ (n = 18). Group 1 was not associated with malignancy or death during the study period, even when symptomatic with seizures or developmental delay, whereas group 2 showed a much more complex picture, requiring individual assessment.ConclusionsFor screening for congenital neurological lesions a single MRI in multiple CMN is a clinically relevant strategy. Any child with a stepwise change in neurological/developmental symptoms or signs should have an MRI with contrast of the brain and spine to look for new CNS melanoma.
Most theorists agree that sarcasm serves some communicative function that would not be achieved by speaking directly, such as eliciting a particular emotional response in the recipient. One debate concerns whether this kind of language serves to enhance or mute the positive or negative nature of a message. The role of textual devices commonly used to accompany written sarcastic remarks is also unclear. The current research uses a rating task to investigate the influence of textual devices (emoticons and punctuation marks) on the comprehension of, and emotional responses to, sarcastic versus literal criticism and praise, for both unambiguous (Experiment 1) and ambiguous (Experiment 2) materials. Results showed that sarcastic criticism was rated as less negative than literal criticism, and sarcastic praise was rated as less positive than literal praise, suggesting that sarcasm serves to mute the positive or negative nature of the message. In terms of textual devices, results showed that emoticons had a larger influence on both comprehension and emotional impact than punctuation marks.
A growing number of studies support the partial compositionality of idiomatic phrases, while idioms are thought to vary in their syntactic flexibility. Some idioms, like kick the bucket, have been classified as inflexible and incapable of being passivized without losing their figurative interpretation (i.e., the bucket was kicked ≠ died). Crucially, this has never been substantiated by empirical findings. In the current study, we used eye-tracking to examine whether the passive forms of (flexible and inflexible) idioms retain or lose their figurative meaning. Active and passivized idioms ( he kicked the bucket/the bucket was kicked) and incongruous active and passive control phrases (he kicked the apple/the apple was kicked) were inserted in sentences biasing the figurative meaning of the respective idiom ( die). Active idioms served as a baseline. We hypothesized that if passivized idioms retain their figurative meaning ( the bucket was kicked = died), they should be processed more efficiently than the control phrases, since their figurative meaning would be congruous in the context. If, on the other hand, passivized idioms lose their figurative interpretation ( the bucket was kicked = the pail was kicked), then their meaning should be just as incongruous as that of both control phrases, in which case we would expect no difference in their processing. Eye movement patterns demonstrated a processing advantage for passivized idioms (flexible and inflexible) over control phrases, thus indicating that their figurative meaning was not compromised. These findings challenge classifications of idiom flexibility and highlight the creative nature of language.
While the basic nature of irony is saying one thing and communicating the opposite, it may also serve additional social and emotional functions, such as projecting humor or anger. Emoticons often accompany irony in computer‐mediated communication, and have been suggested to increase enjoyment of communication. In the current study, we aimed to examine online emotional responses to ironic versus literal comments, and the influence of emoticons on this process. Participants read stories with a final comment that was either ironic or literal, praising or critical, and with or without an emoticon. We used psychophysiological measures to capture immediate emotional responses: electrodermal activity to directly measure arousal and facial electromyography to detect muscle movements indicative of emotional expressions. Results showed higher arousal, reduced frowning, and enhanced smiling for messages with rather than without an emoticon, suggesting that emoticons increase positive emotions. A tendency toward less negative responses (i.e., reduced frowning and enhanced smiling) for ironic than literal criticism, and less positive responses (i.e., enhanced frowning and reduced smiling) for ironic than literal praise suggests that irony weakens the emotional impact of a message. The present findings indicate the utility of a psychophysiological approach in studying online emotional responses to written language.
