Abstrak: Mitigasi Risiko Rantai Pasokan Pemeliharaan Ikan Hias Koi Menggunakan Metode House of Risk. Ketidakpastian adalah suatu kejadian dalam pengelolaan rantai pasokan suatu usaha. Usaha pemeliharaan Ikan Hias Koi juga tidak terlepas dari risiko sepanjang rantai pasokan usaha. Risiko ikan mati disebabkan oleh ketidakpastian pengiriman pakan dan keterlambatan logistik ke pelanggan memerlukan penanganan lebih lanjut. Penelitian ini bertujuan untuk mengidentifikasi, menghitung, dan menganalisis potensi dan penyebab risiko mulai dari hulu sampai hilir rantai pasokan pemeliharaan Ikan Koi sehingga didapatkan sekenario terbaik dalam meminimalisir risiko. House of Risk merupakan pendekatan yang digunakan dalam penelitian ini. Penelitian dimulai dengan identifikasi risk event dan risk agent berbasis Supply Chain Operation Refference, selanjutnya menghitung tingkat severity dan skenario mitigasi. Hasil penelitian ini yaitu teridentifikasi 53 Kejadian risiko dan 33 penyebab risiko dengan 10 prioritas perencanaan guna meminimalisir risiko sepanjang aliran ranti pasok. Kata Kunci : Ikan Koi, HOR, Mitigasi, Rantai Pasok, Risiko Abstract: Risk Mitigation of Koi Ornamental Fish Maintenance Supply Chain Using House of Risk Method. Uncertainty is considerend as challenges as well as opportunities in managing bussiness supply chain. A bussiness has been mitigating a way in order to eliminate such risk through supply chain. As a medium-sized bussiness owner in aquaculture, Mr. Choiruddin have been facing a number of bussinees risks, namely uncertainty of feed delivery and delays to deliver gold fish to customers. This study aims to identify, estimate, and analyze the potential and causes of risk from upstream to downstream of gold fish supply chain in order to figure out the best scenario in minimizing the risk. House of Risk is the approach used in this study. The research began with the identification of risk events and risk agents based on Supply Chain Operation Reference, then calculated the severity level and mitigation scenarios. The results of this study able to identify 53 risk events and 33 risk causes with 10 planning priorities to minimize risk through streams of supply chain. Increasing efficiency and effectiveness is a fundamental problem for every company. Optimization of raw material inventory is an effort for this problem. PT XYZ is a company engaged in manufacturing various flavors of beverages, one of which is sugar. The level of sugar usage in each month is probabilistic and dynamic. PT XYZ's raw material inventory is determined based on field assumptions, causing a shortage or excess of raw material inventory. This study aims to determine the policy of raw material inventory P backorder model with the integration of Monte Carlo simulation and Dynamic System. The raw material inventory policy is determined based on the order time interval, safety stock, and maximum capacity and compares the actual total inventory cost with the Monte Carlo simulation. Dynamic system simulation is carried out to ensure that the P backorder model inventory policy can be applied. The results showed that the total inventory without Monte Carlo was better than the total cost of the Monte Carlo simulation of Rp198.846.582,98 with an order time interval of 0,30929 years, a maximum inventory capacity of 11149,8 kg and a safety stock of 4335,83 kg. From the results of the comparison using the one sample T-test, it was found that there was no statistical difference between the results of the dynamic system simulation and the results from the calculation of the P backorder model so that the P backorder model policy could be applied to PT XYZ to determine the minimum inventory cost.
Bengkel Mobil Dwi Jaya Motor adalah UKM yang bergerak di bidang jasa tepatnya Bengkel Mobil, Bengkel ini melayani servise mulai dari rutin bulanan, onderstail hingga overhole. Permasalahan yang sering ditemukan di Bengkel Mobil Dwi Jaya Motor adalah para karyawan masih kurang waspada dengan adanya bahaya disekitar dan masih jarang menggunakan APD sehingga masih sering ditemukan kecelakaan kerja disaat para karyawan bekerja. Job safety analysis merupakan alat atau tools yang berguna untuk mengukur atau mengidentifikasi sebuah bahaya yang ada pada area kerja dan mencari pengendalian yang cocok dengan potensi bahaya yang ditemukan. Lembar job safety analysis digunakan untuk menganalisa potensi bahaya dengan melakukan penilaian risiko, penilaian risko dengan job safety analysis dilakukan dengan cara wawancara dan observasi langsung kelapanagan dan hasilnya disajikan berupa tabel. Pada tahapan pekerjaan servis up mesin, servis radiator, dan penggantian oli mesin ditemukan beberapa potensi bahaya diantaranya, terkena tumpahan oli panas, terkena radiator yang panas, dan tersembur air radiator. Setelah mengetahui potensi bahaya disarankan kepada pemilik bengkel untuk kedepanya menyediakan APD yang dibutuhkan oleh karyawan diantaranya adalah sarung tangan yang tahan panas, dan kacamata, serta membuat safety sign diarea bengkel.
