The effectiveness of antiretroviral drugs is closely linked to the degree of adherence. The prison environment has specific characteristics that may affect adherence in HIV-positive patients, so that it may not be possible to extrapolate factors associated with nonadherence in HIV+ patients outside prison. The objective was to analyze the prevalence of nonadherence to antiretroviral treatment in three Spanish prisons, and to examine the relation between a large group of factors and nonadherence to the medication. A cross-sectional study of all patients receiving antiretroviral treatment was done. Adherence was evaluated using a validated questionnaire (SMAQ). The prevalence of nonadherence was 54.8%, and the factors independently associated with nonadherence were having difficulties in taking the medication, feeling completely or largely unable to follow the medication, classifying the food as "bad", mentioning not having anyone concerned for them outside prison, suffering anxiety or depression in the last week, and having the flexibility of the prison staff opening their cell in the event of them forgetting their medication.
Resumen: Los datos personales relativos a la salud de un individuo se consideran como datos sensibles, por lo que necesitan una "especial protección". Nos encontramos en una sociedad en la que las tecnologías de información y comunicación (TIC), especialmente en lo referente a las redes sociales, ofrecen amplias posibilidades de intercambio de información médica, sirviendo de apoyo, consejo o ayuda a los compañeros que se encuentran en el mismo ámbito o en cualquier lugar de la geografía mundial. Los avances tecnológicos en comunicación desafían el concepto de "público" y de "privado", de tal forma que la información que se comparte es casi imposible de eliminar, pudiendo llegar a propagarse rápidamente por las redes. Having demonstrated the importance of forensic anthropology in the positive identification of human skeletal remains, we consider it relevant to learn a basic management of the techniques by the students of the Medical Degree, in the context of the subject "Legal Medicine and Ethics". The main objective was to apply theoretical and technical knowledge on physical and forensic anthropology to identify real cases through group work dynamics, concluding with justified decision making. Sixteen 2-hour workshops were organized for groups of 8 students, using skeletal remains of 4 individuals mixed with animal bones. After the human remains were identified, the four skeletons were isolated and an inventory of the available bones was made. The basic biological profile of each of them was analyzed: sex, age and height. These data were entered in a protocol form, which they delivered at the end of the practice. Subsequently, they conducted an anonymous questionnaire of 10 items about the degree of satisfaction on contents, methodology, and utility in their training.With a response rate of 78.4% (n = 125), the highest mean scores were given to items related to: teacher and subject (χ 4,98; Dt. 0.14), expository clarity (χ 4, 94, Dt. 0.24) and adequacy of methodology (χ 4,92, Dt. 0,28). In addition to the favorable results obtained, it was objectified through the protocolized forms, the adequate resolution of real practical cases, based on a joint decision making and justified on the theoretical contents learned.
There is a group of prisoners with specific characteristics that refuse HAART. Specific interventions should be performed in these prisoners to make them aware of the effects of their decision on the course of their disease.
