Penelitian ini dilakukan pada UMKM Nanda Beton yang merupakan industri pengolahan bahan dasar bangunan berupa hiasan beton jenis roster. Tujuan penelitian ini antara lain untuk mengetahui sumber kecacatan pada produk hiasan beton roster dengan menggunakan metode Fault Tree Analysis (FTA) dan Failure Mode and Effect Analysis (FMEA), dan untuk memberikan usulan perbaikan kepada UMKM Nanda Beton. Analisis produk defect pada hiasan beton roster dilakukan dengan menggunakan dua metode analisis yaitu FTA adalah pendekatan analisis kegagalan untuk mengetahui sumber kegagalan pada proses produksi dengan mencari top event kemudian menyusunnya ke dalam pohon kesalahan dan FMEA yang merupakan teknik analisis untuk identifikasi potensi penyebab kegagalan dengan melakukan penilaian untuk memperoleh Risk Priority Number (RPN) yang selanjutnya dilakukan usulan perbaikan. Kedua metode ini digabungkan untuk memperoleh hasil analisis yang lebih baik. Data menunjukkan bahwa terdapat tiga proses produksi yang memiliki jumlah produk gagal (defect) di atas batas toleransi 1% per fungsi produksi, antara lain proses pelepasan cetakan dengan jumlah defect terbanyak sebesar 2,14% dari setiap produksi, proses pencetakan sebanyak 1,77% dari setiap produksi, dan proses pemadatan beton sebesar 1,48% dari setiap produksi. Pada proses produksi terdapat faktor yang mempengaruhi terjadinya kecacatan produk seperti faktor human error maupun faktor alat. Sehingga dilakukan usulan perbaikan berupa pengawasan dan pengecekan secara berkala, penerapan standarisasi terhadap setiap proses produksi dan melakukan pelatihan pekerja serta pemeliharaan terhadap alat produksi untuk menghasilkan produk kualitas baik bebas cacat (defect). Kata kunci: defect, FTA, FMEA ABSTRACT This research was conducted on “Nanda Beton” MSME a building materials processing industry in form of rooster concrete decoration. The purpose of the research is to know the source of a defect in the rooster concrete decoration use Fault Tree Analysis (FTA) and Failure Mode and Effect Analysis (FMEA) methods and also to provide improvement suggestions to “Nanda Beton” MSME. The analysis of defect product in rooster concrete decoration was done using two analysis methods, namely FTA is a fault analysis to identify the source of failure in the production process by looking for the top event, and then arrange it into a fault tree. FMEA is an analytical technique for identifying potential causes of failure by conducting an assessment to get a Risk Priority Number (RPN) then make further improvement suggestions. Both of the methods are combined to get a better analysis result. The data showed that there are three production processes have the number of defect product above the tolerance limit 1% each production function, including the mould release process with the most number of defects 2.14% of each production, the moulding process 1.77% of each production and the concrete compaction process 1.48% of each production. Production process has the factors that influence the occurrence of product defects in each production such as human error factor and equipment factors. So, improvement suggestions needed, there are made the regular supervision and checking periodically, standardization of each production process, conducting worker training and maintenance of the production equipment to produce a good quality product without any defects.
