During a routine dissection course at the University of Muenster (Germany) an unusual course of the lingual nerve was found with entrapment of the nerve between a widely ossified pterygospinous ligament and the medial pterygoid muscle. Furthermore, the nerve's mobility was restricted by a more distal anastomosis with the inferior alveolar nerve. Although incomplete or complete ossification of the pterygospinous ligament is not uncommon, the combination with a medial course of the lingual nerve has not been described before. Besides practical importance for surgeons and anesthetists, the entrapment of the lingual nerve may lead to lingual numbness and pain associated with speech impairment.
In this paper the results of operative and conservative intervention of acetabulum fractures are analysed. It was possible to re-examine 80 of 146 Patients after a period of 5.46 years in mean. 23 patients were treated by osteosynthesis. Time from accident to operation was 13 days in mean. Posterior wall fractures seemed to be the most frequent (classified by Letournel). Results were evaluated according to the following criteria: clinical status (classified by the scheme of Merle d'Aubigne), X-ray, and subjective well-being. In the surgically treated group total hip prosthesis had to be implanted in five patients because of aseptic necrosis of the femoral head, 52.9% of the patients in this group had good results. In the conservative treated cases no total hip prosthesis had to be implanted. 77.8% of the patients had good results. A main aspect is that the average age of the patients who underwent a treatment by osteosynthesis is lower although the fractures were more severe.

Der vorliegende 4. deutsche Konsensus zur immungenetischen Spenderauswahl f?r die allogene Stammzelltransplantation wurde von der Deutschen Gesellschaft f?r Immungenetik (DGI) und der Deutschen Arbeitsgemeinschaft f?r Knochenmark- und Blutstammzelltransplantation (DAG-KBT) gemeinsam erarbeitet und verabschiedet. Er stellt den gemeinsamen Nenner von mehr als 60?Transplantationseinheiten und fast 20 immungenetischen Sucheinheiten dar, die mehr als 3000 deutsche Patienten j?hrlich betreuen. W?hrend der 1. Konsensus im Jahre 1996 den Anfang der Abl?sung von zellul?ren und serologischen Methoden der HLA-Testung durch molekulargenetische Verfahren darstellte, wird nun die hochaufgel?ste Bestimmung der Antigenbindungsstelle der HLA-Molek?le der 5 wichtigsten Genorte als Standard definiert. Das vorliegende Dokument behandelt die Suche nach verwandten und nicht-verwandten Spendern und alle infrage kommenden Stammzellquellen. Bei Sachverhalten, f?r die eine eindeutig wissenschaftliche Evidenz vorliegt, gibt dieser Konsensus eine klare Richtschnur vor, w?hrend es sonst, soweit m?glich, die Handlungsoptionen beschreibt.
