Objective: To evaluate the clinical and radiographic results from 30 cases of revision of total hip arthroplasty using a modular cementless distal fixation prosthesis: the ZMR® Hip System. Method: Between July 2005 and December 2008, 30 operations were performed, on 14 men and 14 women. Two male patients had bilateral surgery. The mean age was 59.2 years (29-81 years), with a mean follow-up of 24 months. The Paprosky classification was used for periprosthetic bone loss, and the Harris Hip Score (HHS) was used to evaluate clinical results. On radiographs, distal migration of the femoral stem was defined in accordance with Sporer. Proximal bone remodeling was classified using the Callaghan criteria. Results: The mean preoperative HHS was 39, and there was a significant increase to 93 points in the final evaluation, which indicated excellent clinical results. No femoral stem migration greater than 5 mm (Sporer) was observed on radiographs, thus suggesting that all the femoral prostheses presented osseointegration and remained stable from the time of the surgery until the final evaluation. The proximal femoral remodeling was either type B or type C in 29 hips, according to Callaghan. Seven patients had complications, but without interfering with osseointegration of the femoral components. Conclusion: Our results from revision of total hip arthroplasty using the ZMR® Hip System were extremely encouraging, and all the components became osseointegrated and remained fixed at the time of the final evaluation.
ObjectiveTo correlate the angles between the acetabulum and the proximal femur in symptomatic patients with femoroacetabular impingement (FAI), using computed tomography (CT).MethodsWe retrospectively evaluated 103 hips from 103 patients, using multislice CT to measure the acetabular age, acetabular version (in its supraequatorial portion and in its middle third), femoral neck version, cervical-diaphyseal and alpha angles and the acetabular depth. For the statistical analysis, we used the Pearson correlation coefficient.ResultsThere were inverse correlations between the following angles: (1) acetabular coverage versus alpha angle (p = 0.019); (2) acetabular version (supraequatorial) versus alpha angle (p = 0.049). For patients with femoral anteversion lower than 15 degrees: (1) acetabular version (supraequatorial) versus alpha angle (p = 0.026); (2) acetabular version (middle third) versus alpha angle (p = 0.02). For patients with acetabular version (supraequatorial) lower than 10 degrees: (1) acetabular version (supraequatorial) versus alpha angle (p = 0.004); (2) acetabular version (middle third) versus alpha angle (p = 0.009).ConclusionThere was a statistically significant inverse correlation between the acetabular version and alpha angles (the smaller the acetabular anteversion angle was, the larger the alpha angle was) in symptomatic patients, thus supporting the hypothesis that FAI occurs when cam and pincer findings due to acetabular retroversion are seen simultaneously, and that the latter alone does not cause FAI, which leads to overdiagnosis in these cases.
Resumo
Objetivo O objetivo deste estudo foi avaliar a associação da anteversão femoral e da força dos músculos do quadril em indivíduos com síndrome do impacto femoroacetabular.
Método Os ângulos de versão femoral descritos nas imagens de ressonância magnética articular e os testes isocinéticos foram avaliados retrospectivamente entre julho de 2016 e dezembro de 2017. Os critérios de inclusão foram: a) versão femoral avaliada pelo mesmo radiologista; b) ângulo α ≥ 55°; e c) ausência de dor limitante durante o teste isocinético. Os picos de torque em flexão/extensão, abdução/adução e rotação interna/externa foram avaliados a 30°/s em 5 repetições. A correlação entre a versão femoral e a força muscular foi avaliada por meio de regressão linear simples, com nível de significância estatística de 5%.
Resultados Um total de 37 mulheres atenderam aos critérios de inclusão. Foram avaliados 51 quadris sintomáticos. Não houve correlação da anteversão femoral nos picos de torque em flexão, extensão, abdução, adução, rotação externa e rotação interna.
Conclusão A anteversão femoral não foi correlacionada à força dos músculos do quadril em mulheres com impacto femoroacetabular sintomático.
