Objectives: This case study reports on one patient presenting atypical and chronic disorders after being diagnosed with Guillain-Barré Syndrome (GBS). Methods: One patient was treated by Yeongseonjetong-eum-gami, Taklisodok-eum-gami, electroacupuncture, and physical therapy. Any improvement in symptoms was assessed by measuring changes in the motor grade of upper limb weakness, the numerical rating scale (NRS) of lower limb numbness, and GBS scores. Results: After 24 days of treatment, the patient's symptoms of GBS showed little improvement. Conclusions: Traditional Korean medical treatment appeared to be effective in reducing GBS symptoms, but more research is required to confirm these results. When treating GBS patients, the clinician should put effort into distinguishing GBS from other nervous disorders, such as acute onset Chronic Inflammatory Demyelinating Polyneuropathy (CIDP).
The purpose of this study is to report the effectiveness of complex Korean Medicine treatment for anterior cruciate ligament (ACL) injuries with meniscus tear. Four patients were treated with complex Korean Medicine by acupuncture, pharmacopuncture and herbal medication. We evaluated the improvement of knee pain and function by Numeric Rating Scale (NRS), Western Ontario and McMaster Universities Arthritis Index (WOMAC Index), EuroQol-5 Dimension Index (EQ-5D Index). After treatment, we found that knee pain was reduced and joint function was improved by NRS and WOMAC index in all cases. In the evaluation of health-related quality of life through EQ-5D index, there was no significant difference in patients with degenerative knee osteoarthritis and severe meniscal injury. This results show that complex Korean Medicine may be an effective option for ACL injuries with meniscus tear.
Objectives This study was performed to research the trends of Korean traditional medicine treatment for cervical spondylotic myelopathy (CSM). Methods Clinical studies about Korean traditional medicine of CSM were searched in 7 online databases. We extracted the characteristics of selected studies according to author, publication year, country, research design, treatment methods, evaluation tools, and adverse events. Results Nineteen studies were selected in online databases. The included studies consist of 8 uncontrolled trial, 7 case reports, 3 randomized controlled trials and 1 non-randomized controlled trials. In these studies, various Korean traditional medicine treatments were performed, and herbal medicine was the most common. The most commonly used outcomes was effective rate. Conclusions This study showed that Korean traditional medicine could be an effective treatment for CSM. It was suggested that various studies should be conducted to provide objective evidence of the therapeutic effects of Korean traditional medicine treatment for CSM.
Objectives The purpose of this research is to evaluate the effectiveness of moxibustion theraphy for frozen shoulder through systematic review and meta-analysis. Methods The effect of moxibustion for frozen shoulder was constructed as a key question to search randomized controlled trials (RCTs) in 16 databases without type of publication restriction. Results A total of 790 studies were searched and 22 RCTs were finally selected through primary and secondary selection/exclusion process. In most of the evaluation indexes used in each study, moxibustion treatment for shoulder pain was statistically significant compared with other treatments. A meta-analysis of effective rate in 10 studies comparing moxibustion with acupuncture and acupuncture showed a statistically significant effect of moxibustion treatment. And there was a significant effect on the improvement of the shoulder mobility, but the effect size of shoulder pain was no significant effect for shoulder pain reduction. In addition in the result of meta-analysis of 4 studies' effective rate comparing moxibustion with electronic acupuncture, the effect of moxibustion was significantly higher than that of electronic acupuncture in frozen shoulder. Conclusions Moxibustion therapy was effective in symptoms in patients with frozen shoulder. However because of the high risk of bias in the quality of the included studies, the applicability of this study seems to be limited. It is thought that future randomized controlled trials will need to be designed and performed by reviewing ways to minimize bias. (J Korean Med Rehabil 2019;29 (3):35-49) 서론»»» 동결견은 서서히 진행되는 어깨 관절의 강직으로 발 현되는 가동범위의 제한과 심한 통증이 동반되는 특징을 가지고 있으며, '오십견', '견관절 주위염', '유착성관절 낭염'의 용어와 통용되어 사용된다. 발병원인은 아직 정확히 밝혀지지 않았으며, 유발 요인으로 외상, 고정, 수술, 당뇨, 갑상선질환 등의 내분비 질환 및 심폐질환 한시훈⋅이기언⋅변다영⋅김영익⋅김민균⋅민부기⋅임한빛⋅정연재⋅오민석 36 J Korean Med Rehabil 2019;29(3):35-49. 이나 뇌졸중 등의 신경계 질환 등이 알려져 있다 1) . 일반적으로 동결견은 1-3년 정도 지나면 자연적으로 증상이 소실되기 때문에 대부분 보존적 치료를 진행하 거나 별다른 치료 없이도 완치가 되는 경우가 많다 2) . 하지만 증상 소실 후에도 동결견 환자의 20-50%에서 견관절 가동 범위 제한 등의 후유증을 호소하며 3) , 6년 간 추적 관찰한 연구에서도 42% 환자들에게서 견관절 운동성이 지속적으로 떨어지는 것이 확인되었다 4) . 동결견의 치료 목표는 증상기에 나타나는 극심한 통 증을 완화시키고 정상적인 관절 운동 기능을 회복시키 고 이를 유지시키는데 있다. 치료 방법은 우선 통증완 화를 위한 스테로이드 주사요법이나 관절 범위 확보를 위한 스트레칭 등의 보존적 치료로 진행되며, 통증 호 전이 없거나 다른 어깨 질환과 동반되어 일상생활이 불 가할 경우 수술적 치료가 고려된다 2,5) . 대표적인 보존적 치료 방법 중 하나인 한의학적 치료 에 있어서도 동결견의 침, 뜸, 약침 및 한약치료 등에 대한 많은 연구가 지속되고 있으며 6-10) , 특히 비침습적 이며 진통 효과가 뛰어난 뜸 치료와 관련된 연구도 꾸 준히 진행되고 있다 11) . 뜸 치료는 한의학적 경락학설을 바탕으로 뜸 연소 시 발생하는 온열자극을 통해 경락에 따뜻한 기운을 불어 넣음으로써 기혈 순환을 촉진하고 기혈을 조화시켜 질 병을 예방하고 치료하는 한의학적 치료법이다. 혈자리 또는 손상 부위에 비침습적인 온열자극과 연소 시 발생 하는 화학적 자극을 통해 조직 세포를 촉진시켜 항염증 세포들을 활성화시키며, 기혈순환을 도와 항염진통 효 과를 나타낸다 12,13) . 동결견 환자를 대상으로 뜸 치료의 효과를 밝혀내는 연구들이 진행되고 있으나, 기존에 진행된 연구들은 단 편적인 결과나 근거수준이 부족한 경우가 대부분이었 으며, 동결견에 대해 뜸 치료가 유의한 효과가 있는지 를 체계적으로 고찰한 연구는 없었다. 이에 본 연구에서는 동결견에 대한 뜸 치료 효과를 나타낸 모든 무작위 대조군 연구(randomized controlled trials, RCT)를 체계적으로 분석하여 뜸 치료의 유효성을 평가하고자 하였으며, 의미 있는 결과를 도출하였기에 보고하는 바이다.
Objectives The purpose of this study is to compare the effects between the KL-Grade and improvement of knee pain treated by Korean Medicine therapy. Methods 114 patients who received inpatient treatment from July 2014 to May 2017 in the Daejeon Jaseng of Korean Medicine Hospital were divided into 5 groups by the KL-Grade. All patients received a combination of treatment including acupunture, pharmacopunture, herbal medication. They were compared and analyzed on the basis of improvement between measuring Numeric Rating Scale (NRS), Western Ontario and McMaster Universities Arthritis Index (WOMAC Index), EuroQol-5 Dimension Index (EQ-5D Index) as they were hospitalized and as they were discharged. The statistically significance was evaluated by SPSS 23.0 for windows. Results After treatment, KL-Grade 0 group's Numeric Rating Scale (NRS), Western Ontario and McMaster Universities Arthritis Index (WOMAC Index), EuroQol-5 Dimension Index (EQ-5D Index) improvement was 2.02±1.69, 7.50±9.67 and 0.11±0.15 respectively. KL-Grade 1 group's improvement was 2.09±1.23, 11.75±13.99 and 0.12±0.13 respectively. KL-Grade 2 group's improvement was 1.60±1.07 and 14.70±14.19 respectively. But In this group, EQ-5D Index has decreased by 0.01±0.10. KL-Grade 3 group's improvement was 1.88±1.31, 7.81±13.35 and 0.13±0.20 respectively (p<0.034). In the case of KL-Grade 4, the population was not statistically significant (N=2) and therefore excluded from statistical significance. And there was no statistically significance between 4 group's improvement after treatment (p>0.05). Conclusions The above study showed that Korean medicine treatments showed significant therapeutic effects on knee pain and degenerative knee joints, but there was no significant difference in the effectiveness of degenerative arthritis (KL-Grade).
