In order to analyze the health risks of insufficient activity by international comparisons, the first author obtained the permission to translate and develop a Taiwan version of the International Physical Activity Questionnaire (IPAQ). The objective was to determine culturally sensitive Chinese translations for the terms "moderate", "vigorous" and "physical activity" as well as to identify representative types of physical activity for Taiwanese. This study used discussions by 12 expert focus groups, 6 expert audits, a scale survey, field study, Cognitive Aspect Survey Methodology (CASM), dual independent translation and back-translation to establish a consensus on physical activity-related concepts, terminologies and types that define the intensity of common activities of Taiwanese by integrating both local and foreign studies. The Chinese terms "fei li", "zhong deng fei li" and "shen ti huo dong", respectively, were identified as suitable and adequate translations for the English terms "vigorous", "moderate" and "physical activity". The common Taiwanese activities were accurately categorized and listed in questionnaires, forming culturally sensitive scales. Taiwan versions of IPAQ's self-administered long version (SL), self-administered short version (SS), and telephone interview short version (TS) were developed. Their content validity indices were .992, .994, and .980, as well as .994, .992, and .994 for language equivalence and meaning similarity between the English and Chinese versions of the IPAQ-LS, IPAQ-SS, and IPAQ-TS, respectively. Consistency values for the English and Chinese versions in terms of intraclass correlation coefficients were .945, .704, and .894, respectively. The IPAQ-Taiwan is not only a sensitive and precise tool, but also shows the effectiveness of the methodology (CASM) used in tool development. Subjects who did not regularly exercise and had an education less than a junior high school level underestimated the moderate-intensity physical activity.

According to the World Health Organization (WHO), the quality of life (QOL) of an individual is essentially defined as the subjective evaluation by such individuals of their own personal life embedded within the context of their culture and values. In the present study, we set out to examine the reliability and validity of the WHOQOL-HIV BREF, a newly-developed, multi-dimensional instrument comprising 31 items designed to assess the QOL of people in Taiwan infected with human immunodeficiency virus (HIV). We collected data on a total of 680 people in Taiwan infected with HIV; these study participants were found to have a mean age of 36.26±10.1 years. Based on our assessment of the psychometric (reliability and validity) properties of the WHOQOL-HIV BREF, we found that the internal consistency (Cronbach's α) ranged between 0.67 and 0.80 across the six domains of physical health, level of independence, psychological health, spirituality, social relations, and environmental health. We also found that the multi-dimensional instrument demonstrated good content, concurrent, and known-group validity, with the results of the construct validity further revealing that the original six-domain structure model was acceptable. The findings of the present study provide strong evidence in support of both the reliability and validity of the WHOQOL-HIV BREF for widespread use in the assessment of quality of life among HIV-infected patients in Taiwan, with the original factor structure of the instrument having been found to be appropriately valid for patients of a Chinese cultural background. We therefore contribute to the evidence on the cultural relevance of the WHOQOL-HIV BREF as a valid measure for cross-cultural comparative studies on QOL.

Objective: This study examined the construct validity, and improved the test reliability and the estimation accuracy for the correlation between domains of the WHOQOL-BREF using multidimensional Rasch analysis. Method: A total of 13,083 adults were administered the 28-item WHOQOL-BREF Taiwan version, which consists of 4 subscales (domains). The multidimensional form of the partial credit model was used to examine the fit of the 4 subscales. For comparison, each subscale individually was also fitted to the unidimensional partial credit model. Standard item fit statistics and analysis of differential item functioning (DIF) were used to check model-data fit. Results: After excluding 2 overall items and deleting 7 DIF items, the remaining items of each subscale in the WHOQOL-BREF constituted a single construct. The test reliabilities and correlations between domains obtained from the multidimensional approach, (0.82-0.86) and (0.79-0.89), respectively, were much higher than those obtained from the unidimensional approach, (0.67-0.75) and (0.53-0.65), respectively. Conclusion: The 19-item WHOQOL-BREF measures more succinct latent traits than the original design. The multidimensional approach yields not only more accurate estimates for the correlation between domains but also substantially higher reliabilities, than the standard unidimensional approach.
