The International Severe Acute Respiratory and Emerging Infection Consortium (ISARIC) COVID-19 dataset is one of the largest international databases of prospectively collected clinical data on people hospitalized with COVID-19. This dataset was compiled during the COVID-19 pandemic by a network of hospitals that collect data using the ISARIC-World Health Organization Clinical Characterization Protocol and data tools. The database includes data from more than 705,000 patients, collected in more than 60 countries and 1,500 centres worldwide. Patient data are available from acute hospital admissions with COVID-19 and outpatient follow-ups. The data include signs and symptoms, pre-existing comorbidities, vital signs, chronic and acute treatments, complications, dates of hospitalization and discharge, mortality, viral strains, vaccination status, and other data. Here, we present the dataset characteristics, explain its architecture and how to gain access, and provide tools to facilitate its use.
Pacientes em uso de oxigenação por membrana extracorpórea (ECMO) apresentam maior risco em desenvolver disfunção de múltiplos órgãos, por isso, a construção de protocolos e treinamento de profissionais se tornam imprescindíveis para diminuição da mortalidade. Pensando nisso, um hospital capacitou enfermeiros para atuarem de forma mais assertiva no cuidado deste paciente. Relatar de que modo ocorreu esse treinamento. Trata-se de um relato de experiência de um hospital privado e filantrópico de grande porte na cidade de São Paulo, sobre a capacitação de enfermeiros para a assistência ao paciente com ECMO. Foram capacitados 52 enfermeiros. O treinamento foi composto por aulas teóricos e práticas, em ambiente de simulação realística, além de discussão clínica, estudo dirigido e debriefing. A capacitação de enfermeiros no cuidado ao paciente com ECMO possibilita melhoria na qualidade e segurança da assistência prestada, prevenindo complicações, garantindo intervenções precoces e melhores desfechos. Training of nurses in patient care with extracorporeal membrane oxygenation (ECMO) Abstract: Patients using extracorporeal membrane oxygenation (ECMO) are at greater risk of developing multiple organ dysfunction, therefore, the construction of protocols and training of professionals become essential to reduce mortality. With that in mind, a hospital trained nurses to act more assertively in the care of this patient. Report how this training took place. This is an experience report at a large private and philanthropic hospital in the city of São Paulo, on the training of nurses to assist patients with ECMO. 52 nurses were trained. The training consisted of theoretical and practical classes, in a realistic simulation environment, in addition to clinical discussion, guided study and debriefing. The training of nurses in ECMO patient care enables improvement in the quality and safety of care provided, preventing complications, ensuring early interventions and better outcomes. Con eso en mente, un hospital capacitó a las enfermeras para actuar de manera más asertiva en el cuidado de este paciente. Informe cómo se llevó a cabo esta capacitación. Se trata de un relato de experiencia en un gran hospital privado y filantrópico de la ciudad de São Paulo, sobre la formación de enfermeros para asistir a pacientes con ECMO. Se capacitaron 52 enfermeras. La capacitación consistió en clases teóricas y prácticas, en un ambiente de simulación realista, además de discusión clínica, estudio guiado y debriefing. La formación de enfermeras en la atención al paciente ECMO permite mejorar la calidad y seguridad de la atención prestada, previniendo complicaciones, asegurando intervenciones tempranas y mejores resultados. RESUMOObjetivo: apresentar a atuação de um time de enfermeiros especialistas na assistência ao paciente submetido à ECMO-VA pós-parada cardíaca. Método: trata-se de estudo qualitativo, descritivo, retrospectivo, na modalidade do estudo de caso, realizado em um hospital filantrópico. Compôs-se a amostra por uma jovem de 18 anos, em que se analisou o prontuário médico e o resultado se apresentou em forma de relato. Resultados: ressalta-se que o estabelecimento de um time bem estruturado, com atuação do enfermeiro na assistência integral contribuíram para evolução do quadro clínico do paciente pós parada cardiorrespiratória resulta em sua recuperação e alta hospitalar. Informa-se que time atuou de forma contínua desde a canulação até a sua retirada, para o cuidado da paciente em ECMO. Conclusão: salienta-se que a atuação do time foi indispensável para um cuidado especializado e um desfecho favorável na assistência à paciente durante todo o seu período em suporte circulatório. Method: this is a qualitative, descriptive, retrospective study, in the modality of the case study, carried out in a philanthropic hospital. The sample was composed by an 18-year-old girl, who analyzed the medical record and the result was presented in the form of a report. Results: it is emphasized that the establishment of a well-structured team with a nurse's role in the integral care contributed to the evolution of the clinical picture of the patient after cardiorespiratory arrest results in their recovery and hospital discharge. It is reported that the team worked continuously from cannulation until its withdrawal, to the care of the patient in ECMO. Conclusion: it should be noted that the team's performance was indispensable for specialized care and a favorable outcome in patient care throughout their period of circulatory support. Few data from Latin American centers on clinical outcomes in coronavirus disease 2019 (COVID-19) patients with acute respiratory distress syndrome who required extracorporeal membrane oxygenation (ECMO) are published. Moreover, clinical and functional status after hospital discharge remains poorly explored in these patients. We evaluated in-hospital outcomes of severe COVID-19 patients who received ECMO support in two Brazilian hospitals. In one-third of the survivors, post-acute COVID-19 syndrome (PACS), quality of life, anxiety, depression, and return to work were evaluated. Eighty-five patients were included and in-hospital mortality was 47%. Age >65 years (HR: 4.8; 95% confidence interval [CI]: 1.4–16.4), diabetes (HR: 6.0; 95% CI: 1.8–19.6), ECMO support duration (HR: 1.08; 95% CI: 1.05–1.12) and dialysis initiated after ECMO (HR: 3.4; 95% CI: 1.1–10.8) were independently associated with higher in-hospital mortality and mechanical ventilation (MV) duration before ECMO was not (HR: 1.18; 95% CI: 0.71–2.09). PACS-related symptoms were reported by two-thirds and half of patients at 30- and 90-days post-discharge, respectively. The median EQ-5D score was 0.85 (0.70–1.00) and 0.77 (0.66–1.00) at 30 and 90 days. Of the 15 responders, all previously working patients, except one, have returned to work at 90 days. In conclusion, in-hospital mortality in a large Latin American cohort was comparable to the Global extracorporeal life support organization registry.
