Patients exposed to a surgical safety checklist experience better postoperative outcomes, but this could simply reflect wider quality of care in hospitals where checklist use is routine.
The integration of molecular and morphological approaches has produced substantial progress in understanding the higher classification of most major invertebrate groups. The striking exception to this is the Polychaeta. Neither the membership nor the higher classification of this group has been robustly established. Major inconsistencies exist between the only comprehensive cladistic analysis of Polychaeta using morphological data and the DNA sequence studies covering all or part of the taxon.We have compiled a dataset of available nearly complete 18S ribosomal DNA sequences and collected an additional 22 sequences (20 Polychaeta in 19 taxa, one Myzostomida and one Phoronida) to obtain more comprehensive coverage of polychaete diversity for this gene. Analyses of the data do not resolve all inconsistencies among current hypotheses of polychaete phylogeny. They do support the recognition (in whole or part) of some clades such as the Eunicida, Phyllodocida and Terebellida that have been proposed on morphological grounds. Our analyses contradict the Canalipalpata and the Scolecida. Although the polychaete sister-group to the Clitellata is not clearly resolved in our analyses, the clitellates are always recovered as a derived clade within the Polychaeta. Increased taxon sampling is required to elucidate further the phylogeny of the Polychaeta.
Background Phylum Porifera includes ∼8,500 valid species distributed world-wide in aquatic ecosystems ranging from ephemeral fresh-water bodies to coastal environments and the deep-sea. The taxonomy and systematics of sponges is complicated, and morphological identification can be both time consuming and erroneous due to phenotypic convergence and secondary losses, etc. DNA barcoding can provide sponge biologists with a simple and rapid method for the identification of samples of unknown taxonomic membership. The Sponge Barcoding Project ( www.spongebarcoding.org ), the first initiative to barcode a non-bilaterian metazoan phylum, aims to provide a comprehensive DNA barcode database for Phylum Porifera. Methodology/Principal Findings ∼7,400 sponge specimens have been extracted, and amplification of the standard COI barcoding fragment has been attempted for approximately 3,300 museum samples with ∼25% mean amplification success. Based on this comprehensive sampling, we present the first report on the workflow and progress of the sponge barcoding project, and discuss some common pitfalls inherent to the barcoding of sponges. Conclusion A DNA-barcoding workflow capable of processing potentially large sponge collections has been developed and is routinely used for the Sponge Barcoding Project with success. Sponge specific problems such as the frequent co-amplification of non-target organisms have been detected and potential solutions are currently under development. The initial success of this innovative project have already demonstrated considerable refinement of sponge systematics, evaluating morphometric character importance, geographic phenotypic variability, and the utility of the standard barcoding fragment for Porifera (despite its conserved evolution within this basal metazoan phylum).
This study reports that there are schizophrenics who do relatively well long term without the routine or continuous use of antipsychotic medication. Specially selected young males undergoing an acute schizophrenic episode were followed, after hospitalization, for up to three years. While hospitalized they were assigned randomly to either placebo or chlorpromazine treatment. Many unmedicated-while-in-hospital patients showed greater long-term improvement, less pathology at follow-up, fewer rehospitalizations and better overall function in the community than patients who were given chlorpromazine while in the hospital. Factors related to post-hospital outcome were good premorbid history and short-lived paranoid characteristics. Considerations which may have an effect on the successful management of acute schizophrenic patients not on medication are mentioned. The findings underline the need for further study of how to utilize antipsychotic medication more selectively in the treatment of schizophrenia.
