Arteriography and phlebography have been used in clinical medicine since their development some twenty years ago. The early work with these techniques was done by dos Santos et al. in 1931, Moniz et al. (1932), and dos Santos (1938). A comparable method of studying the lymphatic circulation would be of obvious value, but its development has been delayed by technical difficulties. The clinical features of chyluria have been described and the ztiology, pathology and treatment reviewed and illustrated by case reports.
The commonest ætiological agent remains filariasis. Chylous reflux due to incompetent megalymphatics and primary lymphatic disease is an established but rare cause.
The importance of full investigation is stressed, especially that of pedal lymphangiography. By combination with pyelography, the site of the fistula may be demonstrated.
Direct surgical approach to the communicating lymphatics is effective, and is now the treatment of choice.
