Objective: to assessed the prevalence of diabetes mellitus (DM) and drug abuse in mothers of children with orofacial clefts (OFC).Methods: 325 women who had children (0-3y) with clefts were interviewed. Data regarding type of diabetes, use of legal/illegal drugs during pregnancy, waist girth and fasting blood sugar at the first prenatal consult were collected.Results: twenty seven percent of the women had DM, out of these, 89% had gestational DM, 5,5% type 1 DM and 5,5% type 2 DM. The prevalence of DM in mothers of children with OFC was 27%, it is significantly higher than the average Brazilian population which is 7.6% (p<0.01) (OR=4.5, 95%CI=3.5-5.8). Regarding drug abuse during pregnancy, 32% of the mothers used drugs and a significant positive correlation was observed between drug abuse and the occurrence of clefts and other craniofacial anomalies (p=0.028) (OR=2.87; 95%CI=1.1-7.4).Conclusions: DM and drug abuse during pregnancy increases the risk for OFC and related anomalies and early diagnosis of DM and prevention of drug abuse, especially in pregnant women, should be emphasized.
ObjectivesTo calculate and compare the nursing staff size associated with self-care promotion at a pediatric semi-intensive care unit.MethodsThis was a prospective study in which 31 children and their caregivers participated. The nursing workload associated with each participant was evaluated at two different times (first and second hospital stays) using the Nursing Activities Score instrument. The first hospital stay corresponded to self-care promotion. Staff size was calculated according to the nursing hours recommended by the Nursing Activities Score instrument and by Conselho Federal de Enfermagem (COFEN) resolution no. 527/16, in the two hospital stays, and the results were compared.ResultsThe nursing workload in the first hospital stay (14.6 hours) was higher than the nursing workload in the second stay (9.9 hours) (p < 0.001). The Nursing Activities Score revealed that according to the nursing hours, the nursing staff size corresponded to 26 and 18 professionals in the first and second hospital stays, respectively, and to 15 professionals according to COFEN resolution no. 527/16.ConclusionThe number of personnel responsible for promoting self-care in pediatric semi-intensive care units, according to the nursing hours suggested by the Nursing Activities Score, was higher than that recommended by the existing legislation. This demonstrates the necessity of reconsidering staff size for this healthcare profile.
Objective:to correlate spirituality and religiosity with quality of life of adolescents with and without cleft lip and palate. Methods:cross-sectional and correlational study involving two groups: case group (n = 40) and comparison group (n = 40). The Duke University Religion Index (DUREL) and the World Health Organization Quality of Life Bref were used for data collection. The Mann-Whitney, Chi-Square, Student’s t-test and Pearson’s correlation tests were used in the statistical analyses, with a significance level of 5% (p ≤ 0.05). Results:organizational religiosity and overall quality of life were significantly higher in the case group (p = 0.031 and p = 0.012, respectively). As for quality of life, the Environment Domain was significantly higher in the case group (p < 0.001). In the correlation between religiosity and spirituality, non-organizational religiosity had a strong correlation (r = 0.62) with organizational religiosity (p < 0.001). In the correlation of religiosity and spirituality with quality of life, only a moderate correlation between spirituality and overall quality of life was identified (r = -0.35, p = 0.026). Conclusion:there was no relationship of religiosity and spirituality with quality of life among adolescents with cleft lip and palate for most aspects evaluated.
SUMMARYPregnancy affects both maternal and fetal metabolism, and even in non-diabetic women, it exerts a diabetogenic effect. Among pregnant women, 2% to 14% develop gestational diabetes. Pregnancy can also occur in women with preexisting diabetes, which may predispose the fetus to many alterations in organogenesis, restrict growth, and the mother, to some diabetes-related complications, such as retinopathy and nephropathy, or to acceleration of the course of these complications, if they are already present. Women with gestational diabetes generally start their treatment with diet and lifestyle changes; when these changes are not enough for optimal glycemic control, insulin therapy must then be considered. Women with type 2 diabetes using oral hypoglycemic agents are advised to change to insulin therapy. Those with preexisting type 1 diabetes should start intensive glycemic control. As basal insulin analogues have frequently been used off-label in pregnant women, there is a need to evaluate their safety and efficacy. The aim of this review is to report the use of both short-and long-acting insulin analogues during pregnancy and to enable clinicians, obstetricians, and endocrinologists to choose the best insulin treatment for their patients. Pacientes com diabetes gestacional geralmente iniciam seu tratamento com dieta e mudanças no estilo de vida; porém, quando essas medidas falham em atingir um controle glicêmico adequado, a insulinoterapia deve ser considerada. Pacientes com diabetes tipo 2 em uso de hipoglicemiantes orais são aconselhadas a iniciar o uso de insulina. Pacientes com diabetes tipo 1 preexistente devem iniciar um controle glicêmico estrito. This study consists in an integrative review of the literature, conducted through electronic search in Lilacs, PubMed, Scopus and Web of Science databases. Articles published in the last five years (2012)(2013)(2014)(2015)(2016)(2017), were selected, primary, available in full, in English, Portuguese or Spanish. The following descriptors were used: Spirituality, religion, quality of life and adolescence, which have been combined with their respective synonyms. A total of 10 articles were included, from which three thematic categories emerged: 1) Favoring and strengthening spirituality and/or religiosity in adolescents; 2) Repercussions of spirituality and/or religiosity referring to the situational confrontation; and 3) Influence of spirituality and/or religiosity in the quality of life of adolescents. There was consensus among the authors studied that there is a strong correlation between religiosity and / or spirituality in the quality of life of adolescents. Resumo Correlação entre espiritualidade, religiosidade e qualidade de vida em adolescentesTrata-se de revisão integrativa da literatura que selecionou artigos primários publicados entre 2012 e 2017, disponibilizados na íntegra, em inglês, português ou espanhol, correlacionando espiritualidade, religiosidade e qualidade de vida de adolescentes. Utilizaram-se os descritores "espiritualidade", "religião", "qualidade de vida" e "adolescente", que foram combinados entre si e com os respectivos sinônimos. Foram incluídos 10 artigos que geraram três categorias temáticas: 1) favorecimento e fortalecimento da espiritualidade e religiosidade de adolescentes; 2) repercussões da espiritualidade e religiosidade referente ao enfrentamento situacional; e 3) influência da espiritualidade e religiosidade na qualidade de vida dos adolescentes.
