Objective: to understand the organization of health practices, based on the interactions at the secondary care level, and to analyze how the actions and services at this level of care contribute to the development of best practice in health. Method: a qualitative approach, based in Grounded Theory. Data was obtained from individual interviews, with managers, health care professionals and health service users making up the sample group representing the secondary level of healthcare. The theoretical model was formulated based on four categories, analyzed based in the elements of the network modeling of health care theoretical framework. Results: The organization of health practices at a secondary level is in the process of consolidation and is contributing to the development of best practices in the locale studied. Conclusion: The broadening of access to consultations and specialized procedures, and the articulation of the network's points, are aspects of this level of care which are considered essential for care which is effective and integral. This study contributes to the analysis of health practices from the perspective of network modeling, based on the interactions between secondary care and the health system's other health facilities, which are shown as going through a process of consolidation in the locale studied. La atención secundaria en salud: mejores prácticas en la red de serviciosObjetivo: Comprender la organización de las prácticas de salud, desde las interacciones en el nivel de la atención secundaria y analizar cómo las acciones y servicios en este nivel han contribuido al desarrollo de mejores prácticas en salud. Método: Enfoque cualitativo, apoyado en la Teoría Fundamentada en los Datos. Se realizaron entrevistas con gestores, profesionales de salud y usuarios, que conforman el grupo muestral de la atención secundaria. Resultados:Se formuló el modelo desde cuatro categorías, analizadas con base en los elementos del modelado de red de atención a la salud. La organización de las prácticas en el nivel secundario se encuentra en proceso de consolidación y ha contribuido al desarrollo de mejores prácticas en salud en el local estudiado. Conclusión: La ampliación del acceso a consultas y procedimientos especializados y la articulación de puntos de la red son aspectos de este nivel considerados imprescindibles para la integralidad del cuidado. Este estudio contribuye para el análisis de las prácticas en salud en la perspectiva del modelado de red, desde las interacciones de la atención secundaria y los otros puntos del sistema de salud, que se presentan en proceso de consolidación en el contexto estudiado.

This study aimed to understand the meanings of care management for undergraduate students, registered nurses and professors during their professional education. It is a qualitative study, based on the Grounded Theory, conducted in a university hospital, between October 2010 and July 2011, with 21 participants in three sample groups (undergraduate students, professors, and registered nurses) and analyzed through Donabedian's reference of health care quality evaluation. The study identified three categories: nurses as managers of material structure and personnel; Nursing management as a dynamic and multifactorial process; Giving to nurses the responsibility for care outcomes and practices and their distance from care practice. This study unveiled the managerial characteristic that has conflicts with the care work process, due to nurses' overload of activities, who are responsible for care structure, process and outcomes conducted by a health team. It is necessary to enhance professional and continuing education to the practice of care management.
Objective: To identify and describe the point of views of healthcare students from the "Federal University of Santa Catarina -Universidade Federal de Santa Catarina (UFSC) regarding their professional preparation to practice in the "Brazilian Universal Health Care SystemSistema Único de Saúde (SUS), the participation of the general population in health care issues and their understanding of the patients bill of rights. Methods: Data were collected using a structuralized questionnaire to complement other qualitative data from 145 undergraduate healthcare students. Results: Findings suggested changes in the profile of the students concerning the healthcare core curriculum. Students reported interest in working for the Universal Health Care System, but they expressed that salary paid by this public health care system may be a disincentive. They perceived that the people have limited financial resources and do not understand the patients' bill of rights. Students support humanization of health care services. Conclusions: Students did not see great value of the participation of the general population in health care issues or social control. This may suggest inadequate professional preparation. Métodos: Como método foi aplicado questionário estruturado, complementar à coleta de dados qualitativos, a 145 estudantes de graduação de cursos da saúde. Resultados: O resultados apontam mudanças no perfil dos estudantes nos cursos adequados às diretrizes curriculares. Destacam interesse em trabalhar no SUS, mas a remuneração é fator impeditivo; percebem os usuários como pessoas de poucos recursos financeiros; desconhecem os princípios da Carta, concordam com a humanização. RESUMO:A inclusão de residentes em enfermagem pode promover mudanças no ambiente do cuidado em âmbito hospitalar. Esta pesquisa objetivou compreender se houve, ou não, mudança nas práticas de enfermagem, a partir da inserção de residentes dessa categoria em uma unidade de internação de Hospital Universitário, na percepção da equipe de enfermagem. Trata-se de pesquisa exploratório-descritiva, de abordagem qualitativa, utilizando a técnica de análise de Bardin e da qual resultaram duas categorias: Perspectiva do profissional técnico de enfermagem e Perspectiva das enfermeiras. Os participantes perceberam não haver mudanças substantivas nas práticas a partir da inserção dos residentes de enfermagem. Foram identificadas situações intermediárias, entendidas como desafios ao Programa. O aperfeiçoamento dos modos de inserção do residente nos cenários de prática poderá contribuir para a educação em enfermagem, o processo de educação permanente em saúde, e para o cuidado prestado nas instituições à sociedade. THE INSERTION OF THE RESIDENT IN NURSING IN A SURGICAL CARE UNIT: PRACTICES AND CHALLENGESABSTRACT: The inclusion of nursing residents can promote changes in the care environment in the hospital ambit. This research aimed to understand whether changes took place or not in the nursing practices, as a result of the insertion of residents of this category in an inpatient unit in a University Hospital, in the perception of the nursing team. This research is exploratorydescriptive, has a qualitative approach, and uses the Bardin analysis technique, from which two categories resulted: The perspective of the nurse technicians and The perspective of the nurses. The participants did not perceive substantive changes in the practices resulting from the insertion of the nursing residents. Intermediate situations were identified, which were understood as challenges to the Program. The improvement of modes of insertion of the resident in the practice settings could contribute to education in nursing, to the process of continuing education in health, and to the care given in the institutions to society.
