Background Amidst the current worldwide epidemic of type 2 diabetes mellitus (T2DM), the global diabetes health burden is projected to reach 522 million in 2030, with much of this increase occurring in developing countries. Aim of the work to evaluate the role of laparoscopic sleeve gastrectomy with loop bipartition (single anastomosis sleeve ileal bypass) as a bariatric and metabolic procedure in control of type 2 diabetes in obese patients. Patients and methods This prospective cohort study included 20 obese adult patients with type 2 DM recently diagnosed within last 5 years. Some of them have other associated comorbidities. They were recruited at department of surgery Ain Shams University. The follow up was obtained during the first year post-operative. Results In this study, complete remission of diabetes was achieved in 75% of the patients by the 3rd post-operative month and in 95% by the end of the study. This was beside marked weight reduction and improvement of lipid profile without causing micronutrients deficiencies during the study period. Conclusion SASI bypass can be one of the most efficient metabolic procedures and could be associated with less risks. The procedure should be considered under investigations until enough long term data are available. Thus it is worth to be explored in research aiming for more data.
The present study evaluated the ability of different extracts of cinnamon and black cumin seeds to inhibit the growth of the phytopathogenic fungus Rhizoctonia solani and its extracellular cell wall degrading enzymes. Concentrations of 300 and 450 ppm of methylene chloride and hexane extracts of cinnamon completely inhibited the growth of R. solani after 72 hours of incubation at 25±1°C in vitro. Methylene chloride or hexane extracts of black cumin seeds at 4000 ppm after 72 hours inhibited the growth of R. solani by 37 and 39% respectively. Moreover, black cumin seed hexane extract at 2000 ppm inhibited 55% and 38% of the activity of pectin lyase (PL) and polygalacturonase (PG) respectively. In addition, the methanolic extract of black cumin seeds at 2000 ppm exhibited a significant reduction of exo-protease activity (74.8%). GC-MS analysis results showed that linoleic acid is the main component of the fixed oil fraction of black cumin seed hexane extract while (E)-cinnamaldehyde is the main component in both hexane and methylene chloride extracts of cinnamon. HPLC-MS analysis of black cumin seeds methanolic extract showed that amentoflavone, was the main component. Docking was used to identify the major component interaction with pectin lyase A and exo-protease.
Background Obesity is nowadays considered as one of the public health problem which is associated with a great variety of diseases, most notably affecting the cardiovascular system. Aim of the Work is to evaluate and compare between the outcome of bariatric surgery directly after Intragastric balloon extraction versus delayed bariatric surgery (six weeks to three months) following balloon extraction. Patients and Methods The study was a Prospective Randomized study which included 30 patients who underwent a planned mini gastric bypass surgery following an intragastric balloon extraction. The patients were divided according to time of surgery to 2 groups: Group A (15patients): Delayed Second scheduled surgery after balloon extraction (6 weeks to 3 months). Group B (15 patients) Immediate surgery in the same session of balloon extraction. Results The mean age of the study was 35.36 years ranging from 21 to 50 years with ±8.227 standard deviation. The mean age of Group A was 35.6 versus 35.13 in Group B. The mean weight of the patients was 112.5±9.8kg ranging from 95 kg to 145kg. The mean weight of Group A was 110.13±8.27 versus 114.86±10.88 in Group B. The mean BMI of the study was 46.46±4.7 ranging from 38 to 60. The mean BMI for Group A was 45.66 versus 47.26 for Group B. Conclusion We concluded that a delayed bariatric surgery following an extraction of the balloon is more favorable than a same session surgery and extraction of the balloon in terms of duration of the operation, hospital stay and early post-operative complications.
