✓ It is commonly believed that in vein of Galen aneurysmal malformations (VGAMs) venous structures normally constituting the deep or Galenic venous system, such as the internal cerebral vein (ICV) and the basal vein of Rosenthal, are not connected to the vein of Galen. In this report, the authors describe 2 cases of successfully treated VGAM in which drainage of an ICV into the vein of Galen was confirmed by follow-up angiography. Two mural types of VGAM were treated using transarterial glue embolization when 1 child was 5 months and the other was 6 months old. The postoperative outcomes for these babies were complete cures. Follow-up digital subtraction angiography obtained after 12 months (Case 1) and 6 months (Case 2) confirmed that the shrunken median prosencephalic vein connects with the deep venous system. The possibility of normal deep Galenic venous drainage must be considered in endovascular management of VGAM. The goal of endovascular intervention is to close only the ventral component of the dilated median prosencephalic vein.
We report the clinical and neuroradiological imaging findings of a 26-year-old man who presented with lumbago related to high flow paraspinal osseous epidural arteriovenous fistulas in the thoracic spine. This case was of particular interest because of his exclusive epidural and paraspinal venous drainage and the presence of a prominent dilated venous pouch in the spinal canal. Angiography demonstrated multiple high flow arteriovenous fistulas with an osseous nidus. Transarterial glue embolization was performed by multistage sessions. Clinical symptoms improved dramatically. The unusual features of this case have important implications for therapeutic management.
[Purpose] In this study, we aimed to analyze customer satisfaction as a tool to
investigate the association among happiness, health status, and well-being using the
Happiness & Health Feeling Scale. [Participants and Methods] We included 17 elderly
participants and measured the happiness and health statuses using the Happiness &
Health Feeling Scale. We analyzed customer satisfaction by correlating subjective
well-being with the questionnaire scores. [Results] The results showed a negative
correlation between subjective well-being and the score on each questionnaire (correlation
coefficient= −0.476). The elderly participants showed lower scores associated with
self-esteem, including external appreciation and self-love, whereas high scores associated
with eating and pleasure. The Cronbach’s alpha was 0.814. [Conclusion] This study showed
an inverse correlation between Happiness & Health Feeling Scale score and well-being,
presumably because of low scores associated with self-esteem, which should be prioritized
for improvement. The additional use of customer satisfaction analysis using the Happiness
& Health Feeling Scale could be helpful to elucidate the subjective association
between happiness and health-related factors.
We analyzed five cases of vein of Galen aneurysmal dilatation (VGAD) and 12 cases of vein of Galen aneurysmal malformation (VGAM) using sagittal T1-weighted images. Tonsilar prolapse (TP) was confirmed in three cases, which were all VGAD. Two of these cases were treated by endovascular procedures. In one of these two cases, elevation of TP was observed after intervention. There were no signs of TP in the control group of 12 VGAMs. Venous hypertension interferes with cerebrospinal fluid (CSF) resorption resulting in increased brain fluid. TP frequently develops in high flow AVFs and is reversible if therapeutic intervention is performed by an experienced clinical team when appropriately indicated. We consider that TP in VGAD is mainly the result of cerebellar swelling due to chronic venous hypertension.
We report a unique case of paravertebral arteriovenous fistulas (PVAVF) presenting with acute epidural hematoma produced by dilated venous pouch. The involved level in this patient makes this an extremely rare case. The main feeding artery was the right thyrocervical trunk and the dilated venous pouch was the posterior internal venous plexus around the upper thoracic spine. The etiology of this abnormal arteriovenous communication remains unclear. This case demonstrates that a venous ectasic pouch comprises important etiological angioarchitecture of radiculopathy and myelopathy in PVAVF.
