Over the last decade, the survival of premature babies has improved dramatically. Such infants, especially those with extremely low birth weight, are still affected by dangerous complications occurring during the neonatal period that often cause brain damage. Intraventricular-intraparenchymal haemorrhage (IVH-IPH), periventricular leukomalacia (PVL), seizures, meningitis and hypoxic-ischaemic encephalopathy are the most common complications. Such problems require more specialized monitoring of brain function during this critical period. In recent years, many studies on very premature infants have shown that aEEG has a high predictive value for both short-term and long-term outcome. In fact, it has been proven that some types of background activity patterns, the absence of a sleep-wake cycle, and seizure activity are related to the onset of early complications such as IVH-IPH and PVL. Most recent studies have shown that an aEEG performed in the early hours or during the first days of life can predict the neurobehavioural development of preterm infants at 2 years and 3 years (Bayley Scale). In particular our study demonstrates that loss of sleep-wake cycling, shown by aEEG, has a high positive predictive value for the development of posthaemorrhagic hydrocephalus (PPH) in preterm infants with IVH; therefore, the study of cerebral background activity and in particular of sleep-wake cycling can be used as an early prognostic tool in patients at risk of PPH.

Rummo, Benevento, ItalySince today the correlation between background cerebral electrical activity at aEEG and brain injury in preterm is unknown. The aim of the present study is to correlate the background activity at aEEG to the patterns of General Movements at writhing age and to avaluate if aEEG is predictive for cerebral palsy.Methods: 17 preterms between 27 and 31 weeks of gestational age with normal head ultrasound were included. The aEEG were recorded during the first 24 hours of life for 60 minutes. The averaged signals were analysed off-line. Background activity was classified into three patterns categories: discontinous low-voltage pattern (minimal amplitude < 3µV), discontinous high voltage pattern (minimal amplitude between 3 and 5 µV ) and continous pattern. General movements were performed during the first 24 hours of life and weekly during writhing age.Results: A background low-voltage pattern correlates positively with Cramped Synchronized General Movements (R: 0.74, p = 0.004), on the other way a high voltage pattern and a continous pattern correlates with normal General Movements (R: 0.67, p=0.006). Conclusions:A discontinous low voltage pattern at aEEG in preterm could be expression of cerebral injury and/or altered maturation. In fact this aEEG pattern highly correlates with pathological Cramped Synchronized General Movements leading to the hypotesis of a similar strong correlation between discontinous low voltage pattern and cerebral palsy in this range of gestational age. High order gestation rates have increased in many western countries in the last decades, which is mostly attributable to a contemporaneous increase in maternal age and infertility treatment. Multiple births have been associated with increased maternal and foetal morbidity and mortality during pregnancy and delivery, including the higher risk for spontaneous abortion, preeclampsia, hydramnios, and maternal haemorrhage. A higher frequency of preterm infants and low birth weight infants have also been reported, as well as a higher rate of malformations, abnormal growth, and trauma at delivery, than in singleton pregnancies. We describe herein the case of six ELBW newborns delivered at 27 weeks and their first year follow-up. All the sextuplets survived and did not have extra uterine growth retardation (EUGR) when discharged from hospital. A developmental delay was detected in five infants at 12 months corrected age (CA), but none of them showed cerebral palsy or severe neurosensorial disabilities.
