Aim Statement. To determine if an electronic EP notification leads to earlier initiation of PC services. Methods. EPs were educated to place a ''Palliative Care Notification'' order in the EMR. They were instructed to order the notification on any patient who they thought would benefit from palliative care including patients they would not be surprised died within in the next 12 months. The notification sent an alert to the PC service for three days a week. On those days, the PC service would reach out to the inpatient or outpatient primary provider to inquire if they agreed to PC consultation. If the primary provider agreed, the PC team would consult. If not, the patient's care would continue as usual.
