(1) Background: The paper addresses the computer simulation and prediction of the service life of the base of removable complete dentures (RCDs) under typical loads caused by biting and chewing food. For this purpose, the finite element method (FEM) was used. It is assumed that various blocks of teeth, such as incisors, canines, premolars and molars, are subjected to cyclic impacts during a human life. (2) Methods: Both symmetric and asymmetric mastication (two- and one-sided loads, respectively) cases were considered. The load level was assumed to be 100 N, which corresponds to the average muscular compression force of typical human jaws. (3) Results: The FEM analysis of the stress–strain state evolution for RCDs under cyclic loads was carried out. Maps of equivalent lines were drawn for the denture base in terms of its durability. A multi-axial criterion was implemented to determine the number of cycles prior to failure by the mechanism of a normal opening mode crack. The FEM-based assessment of the service life of RCDs enabled us to establish the critical stress concentration areas, thereby allowing for further planning for the correction of an occlusal scheme or teeth inclinations. As a result, the service life of RCDs under cyclic loading can be improved. (4) Conclusions: An algorithm for designing RCDs in the case of edentulism based on the FEM simulation using commercial software as part of the procedure is proposed.
Screening studies on dental morbidity of elderly people in the urban and rural population of the Republic of Kazakhstan were conducted. Three age groups were investigated in the districts of Almaty and Almaty oblast: middle-aged (45-59 years old), elderly (60-79 years old) and the oldest (80 years and above). A high loss of teeth was revealed, which was 79.3% in the urban and 90.6% in the rural population of the Republic of Kazakhstan, respectively. Complete absence of teeth on the upper jaw was most common in rural and city dwellers in the oldest and elderly age groups. A high frequency of complete absence of teeth was revealed in elderly rural residents (44.9%). The production of removable prostheses with a double layer base improves the quality thereof. Indications for these prostheses have been determined. In conclusion, the need for orthopedic dental care was the highest in rural areas. Complete absence of teeth on the upper jaw was most common in the oldest and elderly age groups among rural and urban dwellers. A high frequency of complete absence of teeth was revealed among elderly rural residents (44.9%). Orthopedic treatment of persons with complete absence of teeth requires improvement of this assistance. BACKGROUND: The most affordable and economical types of prosthetics are modern designs of completely removable plate prostheses. However, in most cases, they do not meet the patients expectation. OHIP-20-DG, which is a specific quality-of-life (QOL) questionnaire, is a valid tool that allows diagnosis of patients response to treatment, assessment of the effectiveness of prosthetics, and better understanding of patients complaints and dissatisfaction. AIM: To create a sensitive specific questionnaire of dental QOL indicators for patients with total loss of teeth based on the well-known valid questionnaire Dental health impact profile OHIP-49 and to examine patients quality of life before and after prosthetic application. MATERIAL AND METHODS: The QOL of 50 patients with total loss of teeth (17 women and 33 men, aged 4287 years), who received removable dentures with an acrylic polymer base and set teeth, was examined. QOL monitoring was performed using the proposed specific questionnaire OHIP-20 DG. RESULTS: The integral QOL index increased to 47.42% after prosthetic application and to 78.43% following adaptation to the prosthesis after 6 months. Age and gender differences in the QOL level of patients were determined. According to the domains physical discomfort, psychological discomfort, and social maladjustment, men and women assessed their satisfaction with their physical, psychological, emotional, and social functioning differently. Gender differences in patients aged 55 years in the psychological discomfort and social maladjustment domains were significant (p=0.065) when compared with those of patients aged 55 years (p=0.115). CONCLUSION: Results of monitoring the QOL of patients with total loss of teeth before, during the stages, and after application of prosthetics with fully removable dentures, obtained using the OHIP-20 DG questionnaire, allow us to reliably assess the effectiveness of prosthetics.
Aim: To study and systematize the complaints of patients with removable prostheses, paying attention to the clinical manifestation of the torus and its sensitivity, and to determine the effectiveness of the use of two-layer bases in removable prosthetics. Methods: In the clinic of orthopedic dentistry, 104 people were examined and received orthopedic treatment, including 37 patients in the comparison group. The main number of patients up to 55.3, complained of poor fixation and violation of the chewing process. In order to improve the functional efficiency of the removable prosthesis, two-layer bases using soft linings, Gossil (Russia), Furji (Japan), and Mucopren (Germany), are proposed. Clinical and laboratory studies were conducted (Estesmary, gattingeri, physicomechanical studies of soft linings). The effectiveness of using soft pads in removable prosthetics is shown. The conducted research allowed creating a clinical classification of the torus depending on the pain sensitivity of the oral mucosa-type I-painless torus, type II-moderately painful, and type III-painful when touched.
