The article considers the connection of the works of V. G. Korolenko with the literary Western European tradition of depicting prominent people, labelled by the stamp of genius, who left a noticeable mark in science. The concept of genius is considered in the interpretation of philosophers I. Kant, A. Schopenhauer and F. Nietzsche, who saw it as consistent with their theories: contemplation and self-denial, will, energy of all previous generations. According to philosophical concepts, the following main trends in the literature of the nineteenth century were formed: the image of fanatical thieves from the works of M. Shelley, G. Wells, or infantile observers, as the heroes of the works of J. Verne – eccentrics detached from the real life. The realistic tradition in the depiction of images of scientists is considered on the example of the work of A. Chekhov «Boring Story». The image of a scientist created by a writer reflects the main contradiction in the life of a creative person – the discrepancy between life in science and real existence. There wasn’t any difference between creative and real life for V. Korolenko. He «lived as he wrote, and wrote as he lived». The great righteous man of everyday life, a missionary of spiritual deeds, the writer Korolenko created an image of the scientist Izborsky similar to his own life principles. The narrative structure of the work enabled the author to reflect on his student's past and think about his own unrealized future. It is hypothesized that Korolenko could be new Timiryazev or Mendeleev; however he chose the way forward active citizenship and revolutionary mood. The paper examines how the writer transformed the image of the scientist, which developed in European literature, into a new type of hero, due to socio-historical conditions and the spiritual search for time. The images of scientists created in the story «On both sides» and partly in the novel «The Story of My Contemporary» are analyzed from two points of view – the narrator-student and professor-teacher. A young man, who is also in a difficult mind condition, perceives personality, oratory, moral principles of the scientist Izborsky. The analysis of the narrator's structure helps to determine the conceptual features of the image of an ascetic scientist, among which is paid attention to the presence in the works of historical context, the strength of moral influence on students, admiration for the prospects of scientific discoveries.
The aim. To substantiate the effectiveness of the use of VAC-therapy in the treatment of wounds in patients with diabetic foot syndrome. Materials and methods. The patient with a complicated form of diabetic foot syndrome was examined. The monitoring of the course of the wound process is supplemented by clinical, cytological, microbiological and morphological criteria. In the treatment of patients we used vacuum therapy apparatus. Vacuum wound therapy was performed in negative pressure regimens in the range of 80–125 mm Hg. Results. In the pattern of morbidity in economically developed countries, diabetes mellitus (DM) occupies one of the first places. Its prevalence is 1.5–6 %. The number of patients with diabetes in Ukraine is likely to reach 14 million by 2020. Lesions of the lower extremities of different genesis occur in 30–80 % of people with impaired carbohydrate metabolism. Most often, these lesions are complicated by the development of chronic peptic ulcer defects, which, when untimely diagnosed and inadequate treatment, leads to amputation of the affected limb. Even when radical surgery is avoided, the long and costly treatment of trophic disorders of the soft tissues of the feet and legs leads to huge costs of both material resources and time for medical staff. In 15 % of patients with DM, trophic ulcers of the lower extremities are diagnosed. Recent studies have focused on such important aspects of the study of this issue as the early diagnosis of soft tissue lesions in individuals with DM, the development of treatment aimed at preventing amputations, facilitating the further rehabilitation of patients. Vacuum therapy is one of the new methods of treating wound defects, including in patients with diabetic foot syndrome. Topical application of vacuum therapy has already become an integral therapeutic option for wound debridement in patients with diabetic foot syndrome. However, when applying the classic vacuum therapy technique, these patients have the problem of delaying the outflow of tissue exudate and lysis detritus to the vacuum system. Conclusions. Optimization of the technique of the influence of negative pressure on the wound by applying forced drainage with irrigation of the wound with an antiseptic can maintain a constantly high concentration of antimicrobial agent in the wound, increase the speed of wound dialysis and prevent clogging of pores of the sponge and the bacterial colonies in it. The application of this technique avoids high amputation, accelerates the time of wound healing, and retains the supporting function of the lower extremity.
Integration of artificial intelligence in cosmonaut's health monitoring system is a top priority trend in space medicine aimed to enhance crew safety in long-duration space missions. The cutting-edge diagnostic tools aboard the ISS Russian segment such as digital funduscopy, and optic CT of the retina and optic disk allow of piling up statistics of pre- in- and post-flight examinations for calculating the basic block for AI applications. Timely detection of visual dysfunctions in autonomous remote space missions with the help of AI algorithms will increase efficiency of prevention and therapy. Analysis of the experience with convolutional neural networks inophthalmology suggested a CNW architecture for detecting eye grounds pathologies by DF automatic analysis. A developed binary classifier demonstrated sensitivity, specificity and precision of 88.6%, 85.2% and 87%, respectively. In remote missions, the healthcare and therapy system should embody the concept of intelligence telemedicine circuit with artificial neural networks being the pivot. Digital ophthalmology with integrated CNW techniques can be useful also for early diagnostics of eye grounds pathologies within the program of exploration crews screening.
