Indonesia needs mental health service methods that are able to reach a wider community with challenging geographical conditions and a large population. The need is even more urgent with the current situation of physical restrictions in the current pandemic. The increasing number of mental health cases and the limited availability of professional resources that are insufficient to meet the needs, causing mental health disparities. The increasing need for psychological assistance in particular situations is now a challenge for service providers. This paper aims to obtain a scientific basis from several evidencebased studies that show the effectiveness of services using telemental health (TMH) or better known as telepsychology in several community groups and countries. This article is a literature review from various sources through the Google scholar search engine, Google, and PubMed with the keywords telemental health, telepsychology, and telecounseling. The rapid development of information technology in the digital age has become a promising opportunity. With the optimal integration of mental health services and technology, the results obtained can be one of the alternative solutions in reaching the wider community and minimizing existing gaps. The use of two-way long-distance telecommunications media, utilizing telephone and video conferencing with due regard to procedures and professional ethics telepsychology implementation in Indonesia needs to consider several things, including procedures and professional service ethics; infrastructure availability; community readiness and culture; and also, the ability of psychologists as providers of this service. Abstrak Indonesia membutuhkan metode layanan kesehatan mental yang mampu menjangkau masyarakat luas dengan kondisi geografis yang menantang dan populasi penduduk yang besar. Kebutuhan tersebut menjadi semakin mendesak dengan adanya situasi pembatasan fisik di masa pandemi yang dihadapi saat ini. Meningkatnya jumlah kasus kesehatan mental dan keterbatasan ketersediaan sumber daya profesional yang ada tidak cukup memenuhi kebutuhan pelayanan, sehingga menimbulkan kesenjangan kesehatan mental. Meningkatnya kebutuhan pendampingan psikologis pada situasi khusus saat ini menjadi tantangan bagi penyedia layanan. Makalah ini bertujuan untuk memperoleh dasar ilmiah dari beberapa studi berbasis bukti yang menunjukkan efektivitas layanan menggunakan telemental health (TMH) atau lebih dikenal telepsikologi di beberapa kelompok masyarakat dan negara. Artikel ini merupakan reviu literatur dari berbagai sumber, melalui mesin pencari Google Scholar, Google, dan Pubmed dengan kata kunci telemental health, telepsychology, dan telecounseling. Perkembangan teknologi informasi yang pesat di era digital ini menjadi peluang yang menjanjikan. Hasil yang didapatkan, dengan adanya integrasi pelayanan kesehatan mental dan teknologi secara optimal, dapat menjadi salah satu solusi alternatif dalam menjangkau masyarakat yang lebih luas dan meminimalisir kesenjangan yang ada. Penggunaan media telekomunikasi jarak jauh dua arah, memanfaatkan telepon, dan konferensi video dengan memperhatikan prosedur serta etika professional. Implementasi telepsikologi di Indonesia perlu mempertimbangkan beberapa hal, meliputi prosedur dan etika layanan professional; ketersediaan infrastruktur; kesiapan dan budaya masyarakat; serta juga kemampuan psikolog sebagai penyedia layanan ini.
Add psychological interventions based mental health services community for treating people with schizophrenia (ODS) can provide positive opportunities in the recovery process. However, there was no systematic literature review for this yet. This article aims to identify and synthesize ODS's psychological interventions in the community to portray held various interventions. A systematic narrative review with the procedure of PRISMA was used in this research. The inclusion criteria are 1) psychological intervention; 2) involvement of a psychologist; 3) community settings. The search was carried out from April-May 2020. Out of 103 journals, 72 journals were reviewed, and a total of 7 journals were analyzed in this study. The involvement of psychologists in crucial positions in community mental health services is rare. Most of the interventions are led by psychiatrists. Psychological interventions include psychoeducation, CBT, SST, social support therapy, and online psychotherapy with "telementalhealth". Psychosocial interventions are given with pharmacotherapy. There was a positive effect of using psychological interventions for ODS in the community. Intervention on ODS in the community must involve multidisciplinary and multi-professional skills. Psychologists' involvement in psychological interventions can improve clinical outcomes and psychological flexibility to provide satisfaction to patients. It supports recovery-oriented mental health services.
Strengthening integrated and community-based mental health services is essentially needed to address the treatment gap and provide effective services that can be accessed by all groups. To realize this goal, the cross-sectoral involvement that is part of the Tim Pembina, Pengarah, dan Pelaksana Kesehatan Jiwa Masyarakat (TPKJM) is at the forefront because it has direct access to mental disorders cases in their region. This article aims to identify the role of each member of TPKJM in pursuing mental health services. Participants involved in each stage of data collection were members of TPKJM at the sub-district and village levels. The data were collected using three methods: surveys, focus group discussions, and interviews. The collected data were analyzed using an inductive thematic analysis approach to obtain a description of the role of each member of TPKJM within the case handling flow model. Five role themes were identified, including: (a) setting regulations and planning; (b) community clinical services; (c) psychoeducation; (d) safeguarding emergency cases; and (e) administrative advocacy. Regulations regarding the clarity of roles and handling flow are recommended.
Background Recovery-oriented mental health service has become the focus of global change in mental health services. Most of North industrialized countries have adopted and implemented this paradigm in the last two decades. Only recently that some developing countries are trying to follow this step. In Indonesia’s case, there has been little attention to developing a recovery orientation by mental health authorities. The aim of this article is to synthesize and analyze the recovery-oriented guidelines from five industrialized countries that we can use as a primary model for developing a protocol to be implemented in community health centre in Kulonprogo District, Yogyakarta, Indonesia. Method We used a narrative literature review by searching for guidelines from many different sources. We found 57 guidelines, but only 13 from five countries met the criteria, including five guidelines from Australia, one from Ireland, three from Canada, two from the UK, and two from the US. To analyze the data, we used an inductive thematic analysis to explore the themes of each principle as described by the guideline. Result The results of the thematic analysis revealed seven recovery principles, including (1) cultivating positive hope, (2) establishing partnerships and collaboration, (3) ensuring organizational commitment and evaluation, (4) recognizing the consumer’s rights, (5) focusing on person-centeredness and empowerment, (6) recognizing an individual’s uniqueness and social context, and (7) facilitating social support,. These seven principles are not independent, rather they are interrelated and overlap each other. Conclusion The principle of person-centeredness and empowerment is central to the recovery-oriented mental health system, while the principle of hope is also essential to embracing all the other principles. We will adjust and implement the result of the review in our project focusing on developing recovery-oriented mental health service in the community health center in Yogyakarta, Indonesia. We hope that this framework will be adopted by the central government in Indonesia and other developing countries.
