The role opioids play in the development of cancer metastasis and recurrence is far from clear and appears to differ depending on the cancer cell type in question. Prospective randomized controlled trials are currently underway in humans to help clarify the situation further and there results are awaited with anticipation. The negative impact of pain on the immune system is well documented and it appears that appropriate analgesia is paramount in minimizing this. Opioids still constitute a central role in the management of moderate-to-severe cancer pain.

AbstractBackground: Decisions to admit high-risk postoperative patients to critical care may be affected by resource availability. We aimed to quantify adult ICU/high-dependency unit (ICU/HDU) capacity in hospitals from the UK, Australia, and New Zealand (NZ), and to identify and describe additional 'high-acuity' beds capable of managing high-risk patients outside the ICU/HDU environment. Methods: We used a modified Delphi consensus method to design a survey that was disseminated via investigator networks in the UK, Australia, and NZ. Hospital-and ward-level data were collected, including bed numbers, tertiary services offered, presence of an emergency department, ward staffing levels, and the availability of critical care facilities. Results: We received responses from 257 UK (response rate: 97.7%), 35 Australian (response rate: 32.7%), and 17 NZ (response rate: 94.4%) hospitals (total 309). Blue dyes such as Patent Blue V (PBV) have been used in medical procedures for decades, and in the United Kingdom they are routinely utilised in sentinel lymph node biopsy (SLNB) for staging the axilla in early breast cancer. However, it has long been recognised that such dyes are associated with anaphylaxis. It has recently been estimated in a prospective study that allergy to PBV occurs with a frequency of 0.9%. Since repeated SLNB (and therefore further exposure to PBV) is increasingly being advocated for the small proportion of patients who develop a local (in-breast) recurrence, and because anaphylaxis can be life-threatening, it is important that those individuals that are allergic to PBV are recognised on their first medical exposure. The measurement of serum mast-cell tryptase (MCT) and skin prick test (SPT) are used in the investigation of suspected anaphylaxis because positive results are supportive of type-1 mediated hypersensitivity. Here we report the clinical features, MCT results and SPT results that pertain to a series of four patients referred to our drug allergy clinic with suspected anaphylaxis following SLNB. We recommend that all patients that show clinical evidence of allergy following exposure to PBV are referred to a specialist drug allergy service for further evaluation to investigate the cause.
Background: Familiarity with environment, processes and equipment reduces the risk inherently associated with critical care transfers. Therefore, the North West London Critical Care Network decided to create a standardised ideal transfer bag and contents to improve patient safety. Methods: A four-round modified Delphi survey developed a condensed and clinically tested content list. An expert panel then designed an ideal transfer bag based on agreed important principles. Results: Participants completed two rounds of an electronic survey. Round 3 comprised an expert clinical panel review, while round 4 tested the contents over 50 clinical transfers. The prototype bag's design was adjusted after clinical use and feedback. Discussion: This project has introduced a standardised critical care transfer bag across our network. A similar technique could be used for other healthcare regions. Alternatively, the above critical care transfer bag could be adopted or adapted for regional use by clinicians.
Parametric processes play an important role in applications related to laser plasma interaction [P. K. Kaw, Review of Modern Plasma Physics, 1.1 (2017)]. Occurrence of these processes has primarily been reported in the context of laser interacting with an un-magnetized plasma. The regime of magnetized plasma, on the other hand, has remained largely unexplored from this perspective. Recent technological advancements in the production of high magnetic fields [Nakamura, Daisuke, et al., Review of Scientific Instruments, 89.9, 095106 (2018)] bring the area of laser interaction with magnetized plasma targets to the forefront of investigation. In this study, the parametric process of Brillouin scattering for a magnetised plasma target has been demonstrated with the help of one dimensional Particle-in-cell simulations using the platform of OSIRIS-4.0. The external magnetic field has been chosen to be directed along the laser propagation direction. This geometry supports the propagation of right (R) and left (L) circularly polarized electromagnetic waves in the plasma when the laser frequency falls in the appropriate pass band of the respective dispersion curves. A detailed study identifying the scattering process with differing strengths of the applied external magnetic field, and for various polarizations of the incident electromagnetic pulse has been carried out. The conditions favouring the excitation of parametric Brillouin scattering process has been outlined. The nonlinear regime of the scattering process has also been investigated.
