Background: Overweight and obesity are major health concerns worldwide, with adverse health consequences during the life span. This study measured socioeconomic inequality in overweight and obesity among Iranian adults. Methods: Data were extracted from 129,257 Iranian adults (aged 35 years and older) participated in the Prospective Epidemiologic Research Studies in IrAN (PERSIAN) in 14 provinces of Iran in 2014. Socioeconomic-related inequality in overweight and obesity was estimated using the Concentration Index (C n ). The C n further decomposed to find factors explaining the variability within the Socioeconomic related inequality in overweight and obesity. Results: Of the total number of participants, 1.98, 26.82, 40.76 and 30.43% had underweight, normal weight, overweight and obesity respectively. The age-and sex standardized prevalence of obesity was higher in females than males (39.85% vs 18.79%). People with high socioeconomic status (SES) had a 39 and 15% higher chance of being overweight and obese than low SES people, respectively. The positive value of C n suggested a higher concentration of overweight (0.081, 95% confidence interval [CI]; 0.074-0.087) and obesity (0.027, 95% CI; 0.021-0.034) among groups with high SES. There was a wide variation in socioeconomic-related inequality in overweight and obesity rate across 14 provinces. The decomposition results suggested that SES factor itself explained 66.77 and 89.07% of the observed socioeconomic inequalities in overweight and obesity among Iranian adults respectively. Following SES, province of residence, physical activity, using hookah and smoking were the major contributors to the concentration of overweight and obesity among the rich. Conclusions: Overall, we found that overweight and obesity is concentrated among high SES people in the study population. . Accordingly, it seems that intersectional actions should be taken to control and prevent overweight and obesity among higher socioeconomic groups.
Combination of both renewable and fuel-based generation systems is an advantageous approach to develop off-grid distributed power plants. This approach requires evaluation of the techno-economic potential of each source in a selected site as well as optimization of load sharing strategy between them. Development of a remote hybrid power plant in an off-grid area is the interest of this study. Defining all available combinations, characteristics of performance, cost and availability of them evaluated. Applying constraints, multi-objective target domain based on load following and Levelized Cost of Electricity is established in which by utilizing Pareto front approach, optimized scenarios is achieved.
Background: Cystic echinococcosis (CE) is a life-threatening disease in many countries. Albendazole, as the drug of choice for medical treatment of CE, is accompanied by adverse effects and may be ineffective in 20%–40% of cases; hence, new and more effective compounds are urgently needed to optimize the management of the disease. This study was performed to evaluate the efficacy of Zataria multiflora essential oil (ZMEO) versus albendazole against human liver CE. Materials and Methods: In this nonrandomized and single-blinded clinical trial, thirty patients who were infected with liver CE were divided into two groups (15 in each) and treated with albendazole (800 mg daily) and ZMEO (60 mg daily), respectively. Albendazole and ZMEO were administered orally for 180 consecutive days. The volume of hydatid cysts was measured by ultrasonography before and 2, 4 and 6 months after the start of treatment. Simultaneously, biochemical analysis was performed on the blood samples of patients to assess the possible side effects of the two treatment regimens. Results: Two, 4 and 6 months after the start of treatment, ZMEO indicated a significantly higher ability in reduction of the volume of the hydatid cysts, compared to albendazole ( P < 0.05). The mean values of aspartate aminotransferase, alanine transaminase and alkaline phosphatase were also significantly lower in the patients treated with ZMEO in comparison to those treated with albendazole ( P < 0.05). No clinical adverse effects were observed in the patients treated by ZMEO. Conclusion: From the point of view of efficacy and safety, ZMEO indicated a significant superiority to albendazole. Hence, ZMEO may be considered as an alternative for albendazole in the medical treatment of liver CE.
O5] Penetrating missile injuries during the Iraqi insurgency 2006A. Ramasamy * , S.E. Harrisson, M.P.M. Stewart British Military Field Hospital Shaibah, Operation TELIC, Iraq Background: Since the invasion of Iraq in 2003, the conflict has evolved from asymmetric warfare to a counterinsurgency operation. This study investigates the pattern of wounding and types of injuries seen in casualties of hostile action presenting to a British military Field Hospital during the present conflict.Methods: From January 2006 to October 2006, data was collected on all casualties of hostile action (HA) who presented to the sole British Field Hospital in Southern Iraq.Results: Eighty-two casualties presented with penetrating missile injuries from hostile action. Three subsequently died of wounds (DOW 3.7%). Forty-six (56.1%) casualties had their initial surgery performed by British military surgeons. Twenty casualties (24.4%) sustained gunshot wounds, 62 (75.6%) suffered injuries from fragmentation weapons. These 82 casualties were injured in 55 incidents (mean 1.49, range 1-6 casualties) and sustained a total 236 wounds (mean 2.88) affecting a mean 2.4 body regions per patient. IEDs were responsible for a mean 2.31 (range 1-4) casualties per incident.Conclusions: The current insurgency in Iraq illustrates the likely evolution of modern low intensity urban conflict. Improvised explosive devices (IEDs) employed against both military and civilian targets have become the predominant cause of injury. With the current global threat from terrorist bombings, both military and civilian surgeons should be aware of the spectrum and emergent management of the injuries caused by these weapons.
