Neurogenic bladder leading to urinary incontinence has been described in patients of stroke, dementia, Parkinson's disease, and some schizophrenia cases with cognitive impairment possibly due to impaired cortical inhibition of the urinary bladder. The underlying brain abnormalities for urinary incontinence are similar in such cases. We report here such a case of neurogenic bladder responding to treatment with clozapine. The possible mechanism of action and clinical implications are described.
Chronic and recurrent benign pain complaints are common among children and adolescents. Although many young persons with chronic pain adapt well, a small, but significant, proportion experience marked functional deficits. Pain can produce life disruptions, e.g. impeding activities and maturation, and interferes with family functioning. Conventional medically-based approaches have been inadequate in addressing chronic pain and its sequelae. Instead, effective management requires an interdisciplinary approach involving paediatricians and mental health practitioners working collaboratively to treat psychiatric comorbidities, enhance the patientÕs functional adaptation, restore the child or adolescentÕs maturational and social capabilities and reduce family disruptions.

In addition to the assessment of pain frequency and severity, attention needs to be directed at examination of affective states, cognitive appraisals, coping strategies, and functional limitations associated with chronic pain states • Pain reduction and functional restoration can be achieved with pharmacologic, psychotherapeutic, and familybased interventions • The breadth and depth of clinical skills offered by mental health practitioners are essential components to the comprehensive and multidimensional treatment of chronic pain-afflicted youth
In spite of India showing progress in various medical, economic and social fronts, the care of the transgender individual is still encumbered by the various biases and taboos that people hold. But, this was not true in the antiquity. Hindu mythology holds transgender individuals in a status equal to other genders. This brief review of the various references of transgender individuals in Hindu mythology throws light on the various ways the topic of transsexualism and changing gender was addressed.
