BACKGROUND It is unknown whether warfarin or aspirin therapy is superior for patients with heart failure who are in sinus rhythm. METHODS We designed this trial to determine whether warfarin (with a target international normalized ratio of 2.0 to 3.5) or aspirin (at a dose of 325 mg per day) is a better treatment for patients in sinus rhythm who have a reduced left ventricular ejection fraction (LVEF). We followed 2305 patients for up to 6 years (mean [±SD], 3.5±1.8). The primary outcome was the time to the first event in a composite end point of ischemic stroke, intracerebral hemorrhage, or death from any cause. RESULTS The rates of the primary outcome were 7.47 events per 100 patient-years in the warfarin group and 7.93 in the aspirin group (hazard ratio with warfarin, 0.93; 95% confidence interval [CI], 0.79 to 1.10; P = 0.40). Thus, there was no significant overall difference between the two treatments. In a time-varying analysis, the hazard ratio changed over time, slightly favoring warfarin over aspirin by the fourth year of follow-up, but this finding was only marginally significant (P = 0.046). Warfarin, as compared with aspirin, was associated with a significant reduction in the rate of ischemic stroke throughout the follow-up period (0.72 events per 100 patient-years vs. 1.36 per 100 patient-years; hazard ratio, 0.52; 95% CI, 0.33 to 0.82; P = 0.005). The rate of major hemorrhage was 1.78 events per 100 patient-years in the warfarin group as compared with 0.87 in the aspirin group (P<0.001). The rates of intracerebral and intracranial hemorrhage did not differ significantly between the two treatment groups (0.27 events per 100 patient-years with warfarin and 0.22 with aspirin, P = 0.82). CONCLUSIONS Among patients with reduced LVEF who were in sinus rhythm, there was no significant overall difference in the primary outcome between treatment with warfarin and treatment with aspirin. A reduced risk of ischemic stroke with warfarin was offset by an increased risk of major hemorrhage. The choice between warfarin and aspirin should be individualized.
As French populations of the aphid Sitobion avenae exhibit a range of reproductive modes, this species provides a good opportunity for studying the evolution of breeding system variation. The present analysis combined ecological and genetic investigations into the spatial distribution of variation in reproductive mode. Reproductive mode was characterized in 277 lineages of S. avenae from France, and these aphids were scored for five microsatellite loci. The analyses revealed strong geographical partitioning of breeding systems, with obligate asexuals mostly restricted to the south of France, while lineages producing sexual forms were more common in the north. Contrary to what might be anticipated for organisms with frequent parthenogenesis, there was substantial genic and genotypic diversity, even in the obligately asexual lineages. More than 120 different genotypes were detected among the 277 aphid lineages, with an average of 5.9 alleles per locus (range four to 16) and heterozygosity of 56.7%. As with previous studies of allozyme variation in aphids, most loci showed heterozygote deficits, and disequilibrium was common among allelic variants at different loci, even after removal of replicate copies of genotypes that might have been derived through clonal reproduction. Our results suggest that selection is important in structuring reproductive systems and genetic variation in French S. avenae. Canonical correspondence analysis was employed to examine the associations between genotypic and phenotypic variables, enabling the identification of alleles correlated with life-history traits.
This chapter proposes a consensus system for the annotation of Standard German intonation within the framework of autosegmental-metrical phonology: GToBI. First, it provides a survey of existing studies of German intonation, including traditional auditory approaches as well as more recent phonological studies and instrumental analyses. It then gives a detailed exposition of GToBI, showing how the intonation contours considered to be distinctive in the surveyed works can be captured, and compares GToBI to three earlier autosegmental-metrical approaches to German intonation. Finally, it discusses a number of theoretical issues, such as whether pitch accents need to be represented with leading tones or not, how many levels of phrasing are required, and the status and distribution of phrase accents.
a b s t r a c tOxygen transport membranes made of Ba 0.5 Sr 0.5 Co 0.8 Fe 0.2 O 3−ı (BSCF) were manufactured by tape casting and co-firing. The disk-shaped membranes consisted of a top gastight layer (70 m thick) and a porous substrate (830 m thick) with 34% open porosity. The variation of the permeation operation conditions allowed (i) the identification of the different limitations steps in the permeation process, i.e., bulk oxygen ion diffusion, catalytic surface exchange and gas phase diffusion in the membrane compartments and porous substrate, and (ii) the ultimate optimization of the oxygen flux. The variables considered in the systematic permeation study included the inlet gas flow rate of the sweep and air feed, the temperature and the nature of the oxygen feed gas (air or pure oxygen). Moreover, the influence of the deposition of a catalytic activation layer (17 m thick) made of BSCF on top of the thin gastight layer was investigated. As a result of this parametric study, unpreceded oxygen flux values were achieved, i.e., a maximum flux of 67.7 ml(STP) min −1 cm −2 was obtained at 1000 • C using pure oxygen as the feed and argon as the sweep, while a flux of 12.2 ml(STP) min −1 cm −2 at 1000 • C was obtained when air was used as the feed.
