Zusammenfassung Hintergrund Als Ansatzpunkt zur Entwicklung konkreter Optimierungsvorschläge für das HNO-ärztliche Weiterbildungssystem wurde zunächst die Ausbildungssituation an deutschen HNO-Kliniken analysiert, um existierende Schwachstellen der Weiterbildung identifizieren zu können. Methodik HNO-Weiterbildungsassistenten an deutschen HNO-Kliniken wurden eingeladen, an einer Online-Befragung teilzunehmen. Der Fragebogen bestand dabei aus 78 Einzelfragen. Ergebnisse An der Umfrage nahmen 223 Weiterbildungsassistenten der HNO-Heilkunde teil. Defizite der Ausbildung wie ein fehlendes regelmäßiges Feedback zwischen Weiterbildungsberechtigten und Weiterbildungsassistenten/-innen, eine nur mittelmäßig bewertete Vermittlung von Fachkompetenz sowie eine in den Freitextkommentaren genannte „Willkür“ der Weiterbildungsbeauftragten hinsichtlich der Gestaltung des Weiterbildungsverlaufs sowie Zeit- und Personalmangel treten in den Fokus unserer Auswertung. Einige der verbindlich aufgeführten Empfehlungen der Weiterbildungsordnung wie die Festlegung von konkreten Weiterbildungszielen im Rahmen eines regelmäßigen Gesprächs oder der Einsatz eines Log-Buchs werden bislang nur vereinzelt umgesetzt. Forderungen zum Ausbau von externen Fortbildungsmöglichkeiten (Hospitation/Rotation) und einer externen objektiven Überprüfung der klinikinternen Durchführung der Weiterbildung im Sinne einer Qualitätssicherung beispielsweise durch Fachvertreter der Deutschen Gesellschaft für HNO-Heilkunde, Kopf- und Halschirurgie (DGHNOKHC) werden diskutiert. Schlussfolgerung Die Umsetzung einer strukturierten und standardisierten Weiterbildung in Deutschland, welche im Sinne einer freiwilligen Eigeninitiative auch durch die DGHNOKHC in ihrer Umsetzung überprüft wird, kann eine Basis für eine effektivere verbindliche Weiterbildung schaffen.
Only few laryngectomees return to work. However, even before TLE only a third of the patients was employed, another third was unemployed. Most of the patients receive pension retirement after TLE. As return to work is important for many patients, patient consultations should consider possibilities to support vocational rehabilitation before offering to apply for retirement.
BackgroundCollaborations for global surgery face many challenges to achieve fair and safe patient care and to build sustainable capacity. The 2004 terrorist attack on a school in Beslan in North Ossetia in the Russian North Caucasus left many victims with complex otologic barotrauma. In response, we implemented a global surgery partnership between the Vladikavkaz Children's Hospital, international surgical teams, the North Ossetian Health Ministry, and civil society organizations. This study's aim was to describe the implementation and 5-year results of capacity building for complex surgery in a postconflict, mid-income setting.DesignWe conducted an observational study at the Children's Hospital in Vladikavkaz in the autonomous Republic of North Ossetia-Alania, part of the Russian Federation. We assessed the outcomes of 15 initial patients who received otologic surgeries for complex barotrauma resulting from the Beslan terrorism attack and for other indications, and report the incidence of intra- and postoperative complications.ResultsPatients were treated for trauma related to terrorism (53%) and for indications not related to violence (47%). None of the patients developed peri- or postoperative complications. Three patients (two victims of terrorism) who underwent repair of tympanic perforations presented with re-perforations. Four junior and senior surgeons were trained on-site and in Germany to perform and teach similar procedures autonomously.ConclusionsIn mid-income, postconflict settings, complex surgery can be safely implemented and achieve patient outcomes comparable to global standards. Capacity building can build on existing resources, such as operation room management, nursing, and anesthesia services. In postconflict environments, substantial surgical burden is not directly attributable to conflict-related injury and disease, but to health systems weakened by conflicts. Extending training and safe surgical care to include specialized interventions such as microsurgery are integral components to strengthen local capacity and ownership. Our experience identified strategies for fair patient selection and might provide a model for potentially sustainable surgical system building in postconflict environments.
Tracheal agenesis (TA), aplasia or total atresia of the trachea are congenital anomalies which are still incompatible with life. Despite the many attempts of different interventions, there are yet no promising, long-term methods of treatment. Only with sufficient proportion of the proximal or distal trachea available, it is possible to place a tracheostomy, which also opens up new vistas of life for the affected child. In most cases the seldom deformation, trachealagenesis, does not get recognised before the child is born. It may therefore be the immediate diagnosis postnatal that is decisive over the final prognosis of the child. The prepartal suspicion of a duodenal stenosis, an aphonic newborn as well as the frustrane attempts of intubation are possible guidelines of TA. In independence of peripartal and anamnestical factors, individual disciplinary decisions are necessary for further treatments. After the cancellation of intensive care the premature infant of the case report died as consequence of postnatal diagnosed tracheal aplasia. Under circumstances, medical treatments such as the ex utero intrapartum procedure (Exit), the temporary method of extracorporal membrane oxygenation (ECMO) or the use of cartilage tissue for the plastic trachea reconstruction can provide advanced medical opportunities.
