After controlling for parity, maternal and gestational age, singleton pregnancies conceived by GIFT/IVF are at increased obstetrical risk, however the perinatal outcome is comparable despite a lower average birth weight.
In 17 female recreational athletes, ovarian function was monitored using daily hormone measurements and serial ultrasound determinations. Whereas 11 out of 13 women of a control group showed estradiol (E2) maxima beyond 470 pmol/l, progesterone (P4) maxima of 19 nmol/l or more, and a luteal phase length of 9 days or more, only 10 out of 17 athletes satisfied these criteria. Six athletes showed disturbed follicular development, and one athlete showed luteal phase disturbance. Both athletes with disturbed menstrual function (n = 7) and athletes fulfilling the above-mentioned minimal criteria (n = 10) had lower E2 concentrations in all phases of the menstrual cycle (P less than 0.05). P4 concentrations were significantly decreased in the group with disturbed menstrual function (P less than 0.05). Maximal aerobic capacity in the two athlete groups was similar. Neither athlete group showed the expected increase in caloric intake compared with the sedentary controls. It is concluded that recreational running is associated with altered ovarian function. Inadequate nutritional adaptation may be a contributing factor.
The antibacterial activity of human cervical mucus (CM) was examined on standardized microbial colonized agar plates (agar diffusion test). In parallel, the lysozyme content of CM was determined by means of a turbidimetric test system in aliquots of the same CM specimens. Suspensions of living lyophilized Micrococcus lysedeikticus were used as bacterial substrate. Testing was performed in a total of 133 CM samples, obtained at mid-cycle from sexually active women from unselected infertile couples with a median age of 30 (range 21-42) years. All mucus specimens showed considerable antibacterial activity with clearly visible circular inhibition zones around the CM-filled holes in the colonized agar plates. Related to the effect of hen's egg white (HEW)-lysozyme on the same plates, the median activity of the CM specimens in the agar diffusion test was equivalent to 33.0 (range 6.4-391.4) microg/ml HEW-lysozyme. However, there was a wide inter-individual range of antibacterial effects of cervical secretions. The cervical index did not significantly influence the outcome of either test. The pH of the endocervical CM also was not correlated with the antibacterial effect. Sexual activity leading to the presence of spermatozoa in CM considerably increased its antibacterial effect. The activity was markedly higher in samples obtained within hours after intercourse compared with those taken after sexual abstinence of >/=5 days (P < 0.05). In microbially colonized CM specimens compared to sterile CM, all obtained under hormonally standardized conditions, the antibacterial activity in the agar plate test was significantly lower (P < 0.05). The results of this pilot study demonstrate the considerable antibacterial activity of human CM.
ABSTR ACTBackground Infertility patients often have high stress levels which, in some cases, represent a risk of developing depression or anxiety. The SCREENIVF questionnaire is a validated tool to evaluate such risks. Some coping strategies have been shown to be correlated with infertile couplesʼ levels of stress. Determining which strategies are correlated with higher levels of risk for depression or anxiety could be useful to offer targeted psychological counseling to reduce the risk of depression or anxiety.Materials and Methods A total of 296 women and men who attended the Fertility Center at Heidelberg University Hospital completed the SCREENIVF questionnaire and the COMPI coping scales.Data were analyzed first on an individual basis and focused on the couple, using the Actor Partner Interdependence Model.Results On an individual level, active avoidance coping was positively correlated with a higher risk of depression or anxiety in women, while meaning-based coping was negatively correlated with risk in men.When the results of couples were viewed together, women and men using active avoidance coping exhibited higher risk scores as individuals (actor effect), as did their partners (partner effect). Women who used meaning-based coping had positive actor and partner effects.Women using active-confronting coping had a negative partner effect (higher risk score for men).Conclusions These findings indicate that some coping strategies may have a protective effect while others may increase the risk of emotional maladjustment in infertile couples. Further analysis of coping strategies could help to identify new counseling approaches for infertile patients.
