Before starting this study, we hypothesized that forward advancement of mandible with PMA night best act to dilate hypopharynx manifested as increase of PAS.3 However, ultrafast MRI of upper airway during sleep showed an improvement not only of GP obstruction, but also of velopharyngeal obstruction in most subjects. Concerning these results, velopharyngeal dilatation brought about by inferior and forward advancement of the mandible manifested as the change of MP-H, N-Me and S N B and the decrease of intraesophageal negative pressure loading was thought to act cooperatively to suppress velopharyngeal collapse. T o a certain extent, elevation of muscle tonus when equipped with PMA might contribute to maintain patency of upper airway. Considering that VP obstruction is most common in OSAS,' PMA treatment niight be widely acceptable for the disorder.
AbstractWe studied the prevalence of sleep-related respiratory disorders (SRRD) in 101 schizophrenic inpatients (64 men and 37 women) and in 48 healthy volunteers (control group: 22 men and 26 women) using ambulatory pulseoximetric devices. Those with a desaturation index (DI) 2 5 were classified as having SRRD. The prevalence of S R R D in the schizophrenic patients (men 21.9%, women 13.5%) was not higher than that in the control group (men 30.7%, women 13.6%). Gender difference in the prevalence of S R R D was not observed in schizophrenic group. This was probably because the schizophrenic women took an increased amount of sleeping pills. Neuroleptics were shown to be least effected on SRRD.
A case of leiomyosarcoma that appeared to have originated in the ligamentum teres hepatis of a 66-year-old female has been presented. The case was suspected of leiomyosarcoma originating in the ligamentum teres of the liver from the findings by preoperative examinations, and definitely diagnosed as such in a laparotomy and by histological examination of the resected tissue specimen.
