BackgroundIn this study we evaluated the performance of bovine pericardium preserved in glutaraldehyde used as a vascular patch.MethodsFourteen young pigs, six females and eight males, weighting 10.3 - 18.4 kg were used in our study. We implanted three remnants in each pig, two in the abdominal aorta and one was juxtaposed to the peritoneum. The smooth face (SF) and rough face (RF) of each remnant were implanted turned to the vessel inner portion and one remnant was juxtaposed to the peritoneum. The animals were sacrificed in 4.5 - 8 months after surgery (75 - 109 kg). The remnants were assessed for aorta wall, fibroses formation in inner apposition and calcification related to the face turned to the vessel inner portion.ResultsThe rough face showed a lower dilatation level compared to the face implanted in adjacent aorta. There was no difference between intensity and/or incidence of graft calcification when the superficies were compared. The bovine pericardium preserved in glutaraldehyde did not show alterations in its structure when implanted with different faces turned to the inner portion of vessel.ConclusionWhen turned to the inner portion of the vessel, the rough face of the remnant presented a lower dilatation in relation to the adjacent aorta and a better quality of endothelium layer and did not show a difference between intensity and/or incidence of graft calcification.
O comportamento do pericárdio bovino, preservado em glutaraldeído, na confecção de próteses valvulares é bem conhecido. Embora amplamente empregado na forma de retalho, foi pouco estudado neste sentido. Com esta finalidade, 21 cães foram submetidos ao implante cirúrgico de um retalho padronizado de pericárdio bovino substituindo, parcialmente, as paredes da aorta, do átrio esquerdo e do pericárdio. Os cães foram, casualmente, separados em três grupos, segundo o tempo de reoperação. O Grupo 1 com 6 cães reoperados entre 33 e 43 dias; o Grupo 2 com 7 cães reoperados entre 120 e 165 dias e Grupo 3 com 8 cães reoperados entre 225 e 305 dias. Os exames micro e macroscópico evidenciaram: 1) a superfície rugosa do retalho do pericárdio bovino aderiu às estruturas vizinhas, enquanto a superfície lisa pouco aderiu ao epicárdio; 2) o retalho de pericárdio bovino não sofreu alteração estrutural, independentemente da localização de implante; 3) a área final de remendo atrial esquerdo foi significantemente menor do que a dos remendos aórtico e pericárdico, para os cães dos Grupos 2 e 3; 4) a área do remendo atrial diminuiu significantemente, enquanto da aorta e do pericárdio não sofreram alteração com o tempo; 5) a espessura do remendo do pericárdio foi significantemente menor do que a dos remendos da aorta e átrio esquerdo, para cães do Grupo 3; 6) ocorreu a formação de uma camada de tecido conjuntivo fibroso na superfície lisa dos retalhos implantados no átrio esquerdo e na aorta. A espessura do tecido de aposição interna foi significantemente maior no átrio esquerdo do que na aorta, nos Grupos 1 e 2; 7) o tecido de aposição interna dos remendos atrial e aórtico sofreu calcificação nos cães dos Grupos 2 e 3; 8) o tecido de aposição interna dos remendos atrial e aórtico sofreu neoformação de fibras elásticas que aumentou nitidamente com o tempo de implante. Diante do achado, concluímos que o comportamento do pericárdio bovino preservado em glutaraldeído, na forma de retalho, para a confecção de remendo, em cirurgia cardiovascular depende: 1) da superfície de contato; 2) da tensão a que é submetido; 3) do contato com a corrente sangüínea.
The bovine pericardium, preserved into glutaraldehyde, in making valvar phostheses, is well known. Although largely used in the form of scrap, this was little studied. With this intention, 21 dogs were submitted to surgical patch of a standardized bovine pericardium scrap partially substituting, the aorta left atrium and pericardial walls. The dogs were randomly separated in three groups according to the reoperation time. Group 1 with 6 reoperated dogs between 33 and 43 days; group 2 with 7 reoperated dogs between 120 and 165 days and group 3 with 8 reoperated dogs between 225 and 305 days.
