The involvement of iron and inflammation parameters on overall survival in non-small-cell lung cancer (NSCLC) patients was studied. Furthermore, transferrin receptors 1 (TfR1) and ferritin expression in tumor tissue, tumor stroma, and normal lung tissue were analyzed. Iron metabolism and inflammation parameters were determined by automated laboratory measurements at the time of diagnosis. TfR1 and ferritin expression were determined by immuno-histochemical methods. About 50% of patients survived 12 months only. At the time of diagnosis more than half of the patients had anemia and significantly elevated serum ferritin. Iron content of serum ferritin (ICF) was below the reference values in 90% of patients. Furthermore, ICF showed positive correlation with iron metabolic parameters and survival but negative correlation with serum ferritin and ESR. The expression of TfR1 and ferritin in tumor cells was observed in 88% or 62% of patients, respectively. Tumor stroma was TfR1 negative and sporadically ferritin positive. Tumor tissue ferritin expression showed negative correlation with serum iron and hematokrit (Ht), and positive correlation with ferritin, erythrocyte sedimentation rate (ESR), alpha-1 globulin, and alpha-2 globulin. Positive correlation was found between TfR1 expression in tumor tissue and alpha-globulin. The correlation between TfR1/ferritin expression in tumor tissue and ICF or survival was not observed. Therefore, we conclude that elevated serum ferritin in sera of NSCLC patients is the result of inflammation and oxidative stress rather than body iron overload. Higher expression of ferritin in tumor tissue may be the consequence of iron deficiency or local toxicity induced by environmental factors.
Neonatal BCG vaccination reduces the risk of tuberculosis and provides protection higher than 80% against the development of meningeal and miliary tuberculosis in newborns. Tuberculosis meningitis remains a major problem and also an important cause of death in some countries. In countries with high and moderate incidence of tuberculosis, prevention from the most severe complications of tuberculosis can be achieved only with a high coverage of the universal BCG neonatal immunization, being higher than 98% in the cohort of newborns. The decrease in BCG immunization coverage within immunization program during the year 2003 in Bosnia and Herzegovina influenced the increase in tuberculous meningitis. During 2002, when coverage with BCG vaccination in cohort of newborns was 90%, the incidence rate of tuberculous meningitis was 19. 04%oo. With the 68% decrease in BCG immunization coverage in the cohort of newborns in Bosnia and Herzegovina during the year 2003, the incidence of tuberculous meningitis raised to 33 33%oo. It has been proven that the 22% decrease of the neonatal BCG immunization coverage in the cohort of newborns /vaccination program of children/ caused 175 times higher number of the tuberculous meningitis cases. Newborns affected by the tuberculous meningitis were not BCG vaccinated. BCG vaccine provided effective protection against tuberculous meningitis, as well against the death of newborns caused by tuberculosis.
Depression is highly prevalent in cancer patients. Variations in intensity and frequency of depression in cancer patients may be attributed, in part, to differences in personality dimensions. Our aim was to asses if dimensions of temperament and character could predict depression in lung cancer patients. Ninety newly diagnosed non-small cell lung cancer patients were assessed in the oncology unit with the Centre for Epidemiologic Studies Depression Scale (CES-D), pain subscale of the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ) C30 and the Temperament and Character Inventory (TCI). Fifty out of 90 patients (55.6%) fulfilled the cut-off criteria for depression on the CES-D scale. Logistic regression performed to assess if depression was predicted by personality dimensions, revealed dimension of temperament Harm Avoidance and pain as significant predictors of depression. Depressive reactions are highly prevalent in lung cancer patients and related to patients' personality. These findings may be helpful in planning preventive, as well as psychoeducational and treatment programmes for newly diagnosed, and depression prone patients.
Tuberculosis (TB) is one of the oldest, the most expanded and the most lethal diseases in human history. Although Koch’s discovery of the TB causative agent (1882) represented a great progress in the fight against this infectious disease, it took a significant amount of time to reduce morbidity and mortality in the world. In Bosnia and Herzegovina (B&H) it was necessary to implement actions in the form of education where the popularization of measures of recognition, treatment and preventing the disease was done. After the Annexation of B&H to Austro-Hungarian (1908) began an organized fight against TB. Dr. S. Kukric was a particularly prominent individual, putting his effort by working in clinics, through his lectures and numerous popular research papers on tuberculosis. He was followed by many colleagues working inexhaustibly while facing the high incidence of TB and the difficult social situation in B&H. Although this disease is old, at the end of the 20th century a new TB appeared, with new challenges and new, even grater problems. Significant achievement and great progress in the treatment and control of TB infection was achieved by implementing the direct observed short course treatment (DOTS from 2006.) Still, there is a too high incidence of TB that becomes again a serious threat, together with new problems and difficult economic and social situation. Nowadays in Federation of B&H the guidelines of World Health Organization on reinforcing the DOTS strategy are being daily implemented, including multidrug-resistant (MDR) TB and infection control.
This study investigated mortality rate of lungs carcinoma in professional vineyard sprayers. Clinical investigation was performed in 187 professional vineyard sprayers who had been exposed to the inhalation of the particles of Bordeaux mixture for 24 years on average. Bordeaux Mixture is used for prevention against mildew attacking vineyards. The control group was composed of 187 inhabitants of the same area who did not have any contact with the mentioned substance. A cytological investigation of the sputum specimens obtained from 104 tested inhabitants was performed. The sputum specimens were stained with standard haematoxylin-eosin method and also with special method (rubeanic acid) to prove the incidence of copper granules in macrophages. The findings show a considerable statistical difference in the frequency of occurrence of lungs carcinoma between the group of vineyard sprayers smokers and the control group (X2=4.77,p<0.01). The risk of lungs carcinoma in the vineyard sprayers was three times higher compared to the risk of smokers in the control group, with a statistical probability of 95% in the scope from 1.16
