There was clinical observation of the treatment of traumatic prosthetic stomatitis in 44 people (9 men and 35 women) aged 61 and 82 years suffering from 'dry mouth' syndrome due to radiation and (or) drug sialoadenopathies developed after combined treatment of malignant neoplasms of the organs of the oropharyngeal zone. Using semi-quantitative method, a comparative evaluation of the effectiveness of a new drug based on chelate organic germanium guanine complex, sodium alginate and xylitol (main study group) and previously used drug (control group) for local therapy of traumatic lesions of the oral mucosa was made. It was found that on day 9 in the main study group the effectiveness of treatment was significantly higher (95.0%) than in the control group (80.8%).
Conducted dynamic observation of 69 patients elderly (61-85 years) suffering from chronic diseases of the salivary glands, which were accompanied by pronounced changes in saliva formation and physiological status. The patients were divided into 3 groups: sufferingfrom various malignant tumors of the major salivary glands (n = 21), valsami - disease Mikulic and Sjogrens syndrome (n = 16), drug or radiation sialadenopathy (n = 32). At the 1st stage the patients were given conventional treatment from a dentist, which was directed on prevention of occurrence and progression of inflammatory degenerative distroficski processes in tissues of the salivary glands, as well as improvement of salivation - used peptide bioregulator the vezugen, biocorrectors food alhilal and lesmin during the month. Course this treatment was repeated 4 times a year, i.e. quarterly. At the 2nd stage of treatment (after 6 months after 1-stage) patients of all study groups in addition to this therapy was administered anxiolytic Afobazol 4 weeks. Patients were followed for a year from the date of completion of the 2nd phase of integrated treatment. In the course of the treatment was assessed the physiological status ofpatients. To assess the level of daytime stress and nighttime sleep quality, used instrument-hardware complex on the basis of First Beat sensor and Body Guard, the principle of which is based on the analysis of heart rate variability. It was found that all patients suffering from malignant tumors of the salivary glands, as well as valsami, medical and radiation sialadenopathy indicate a high level of daily stress compared to the control group and deterioration in the quality of nighttime sleep. Upon completion of the 1st stage of therapy in patients with chronic diseases of the salivary glands, clinically marked improvement, but no significant changes in the indicators of the level of daily stress and quality of night sleep did not happen. Use at the 2nd stage of treatment in addition to applied therapy anxiolitika Afobazol was effective and resulted in a 19-49. 3 percent to improve treatment outcomes, reduce daily stress and improve nighttime sleep in these patients that have a positive impact on their quality of life.
Presents information from the life of a maxillofacial surgeon and dentist, one of the patriarchs of domestic dentistry, a veteran of the Great Patriotic War, academician of the Academy of Medical Sciences of the Union of Soviet Socialist Republics, doctor of medical sciences, professor, colonel of medical service Anatoly Ivanovich Rybakov, known in our country and abroad. , who was the first director of the Central Research Institute of Dentistry. The data on the formation of A.I. Rybakov as a military doctor and dentist, his combat path during the Great Patriotic War. Turning to the life and professional and social activities of Anatoly Ivanovich, one can find not only interesting scientific facts from the history of dentistry and maxillofacial surgery, but also analogies with modernity, answers to many clinical problems of the specialty and medicine of today. The main directions of his scientific activity, which touched questions of almost all sections of dentistry and maxillofacial surgery, are noted. A.I. Rybakov was one of the creators of the working concept of the pathogenesis of dental caries and periodontal diseases, the author of the first recorded discovery in dentistry on the phenomenon of the production of intestinal antigen by the oral mucosa of mammals, the founder of the study of the epidemiology of dental diseases in Russia and the Republics of the Soviet Socialist Republics. It is noted that he was a refined connoisseur of the history of national ballet, a true professional and an authority in this field of art. Anatoly Ivanovich, being one of the founders of domestic dentistry, as well as the school of dentists and maxillofacial surgeons, is rightfully recognized as one of the patriarchs of domestic dentistry.
Pathology of an eruption of wisdom teeth is importance for a military odontology because the vast majority of the military contingents on age coincides with time of their physiological eruption (16-40 years). In work, on the basis of clinic-radial methods of a research, features of a teething of wisdom at young people of military age are studied and analyzed 3D-tomograms and orthopantomograms of 325 servicemen aged from 18 up to 27 years. During work estimated existence of wisdom teeth on top and lower jaws, extent of their eruption, existence of a retention (or dystopias) and also an adentia or loss of wisdom teeth. It was established that most often eruption of the third molar teeth of jaws at recruits occurs at the age of 23-27 years, at the same time at a series of recruits eruption of the lower wisdom teeth usually is followed by various complications, such as pericoronitis, an acute purulent periostitis of a mandible, a false «acute periodontitis» of the lower second molar tooth that becomes perceptible at mesial shift of a wisdom tooth and dense contact of its coronal part with a distal root of the second molar tooth. The essential value for a full-fledged teething of wisdom has their situation in an alveolar process (part) of a jaw, and further - in dentition. It in many respects defines a clinical picture of the shown eruption complications and also tactics of stomatologic treatment - orthodontic treatment, surgical treatment or their combinations. The infectious and inflammatory complications bound to the complicated their eruption (85,93% of cases) served as the reasons of an exodontia of wisdom. In 14,07% of cases wisdom teeth on both jaws were extracted in a planned order in connection with the forthcoming orthodontic treatment on elimination of dentoalveolar anomalies. Thus, terms and features of a teething of wisdom at recruits need to be considered in clinical practice of a military odontology as their military service can pass in specific living conditions and activity of troops and health service, and at complications of a teething of wisdom the acute stomatologic surgical management, including stationary is in most cases necessary.
