Based on the results of population survey, the prevalence of lower urinary tract symptoms in male population of the Russian Federation North-Western Region was studied. It was established that the urinary disorders severity and life quality reduction ratio associated with these disorders depend on the intensity of the metabolic syndrome presentations, such as diabetes mellitus, arterial hypertension, obesity, and are closely linked to the presence of erectile dysfunction. Keywords: lower urinary tract symptoms; metabolic syndrome; erectile dysfunction.
Introduction. Sexually transmitted diseases (STDs) can potentially disrupt male fertility; however, the prevalence data for STDs among men from infertile couples is controversial. Aim. The aim of the study was to estimate the prevalence of STDs among men from infertile couples seeking medical assistance in an assisted reproductive technology clinic. Materials and methods. We retrospectively studied the medical records of 307 men (mean age: 35 ± 15 years) consecutively attending an outpatient reproductive clinic for infertility treatment. Duration of the infertile period varied from 4 to 228 months (mean: 38 ± 12 months). We collected urethral swab specimen to investigate the presence of Neisseria gonorrhoeae, Trichomonas vaginalis, Chlamydia trachomatis, Mycoplasma, and Ureaplasma species in 235 (76.5%), 232 (75.6%), 270 (87.9%), 301 (98.1%), and 300 (97.7%) men, respectively. Blood tests for syphilis, hepatitis B and C, human immunodeficiency virus (HIV), human herpesvirus (HHV), and cytomegalovirus (CMV) were conducted in 229 (74.6%), 224 (73%), 227 (73.9%), 230 (74.9%), 233(75.9%), and 232 (75.6%) men, respectively, and statistical analysis of the data was performed. Results. Remarkably, all tests were negative in 94 (30.6%) men, and no patient had T. vaginalis or C. trachomatis infection. However, N. gonorrhoeae, Mycoplasma, and Ureaplasma were detected in 1 (0.4%), 6 (2.6%), and 14 (4.7%) men, respectively. Syphilis, hepatitis B and C, HIV, HSV, and CMV tests were positive in 1 (0.4%), 2 (0.9%), 6 (2.6%), 1(0.4%), 177 (76%), and 133 (57%) men, respectively. Conclusions. The prevalence of STDs is high among men from infertile couples. Further studies are warranted to investigate the relationship between STDs and male infertility. Surgical methods of treatment of Peyronie’s disease by plication corporoplasty proved to be effective in the near and distant postoperative periods in the examined group of patients. These methods made it possible to quickly achieve the normalization of the shape of the penis without affecting the erectile function which enabled for patients to restore copulative activity and improve the quality of life.
Introduction. Simple kidney cyst is a common disease that may need surgical treatment for pain, hypertension and kidney obstruction. Long term results of kidney cyst treatment need further investigation. The aim of the study was to assess patient complaints and kidney ultrasound sonography data in patients in a long term after percutaneous cyst aspiration (PCA) with sclerotherapy and laparoscopic decortications of the cyst wall (LDC). Patients and methods. In this retrospective study clinical data of 891 patients (277 (31,1%) men and 614 (68,9%) women, mean age 61,8 years) with simple kidney cysts operated by PCA and LDC (754 (84,6%) and 137 (15,4%) patients respectively) due to back pain, hypertension, big cyst size and obstruction of urine flow from the kidney was analyzed. Control survey and ultrasound sonography scans were performed in 80 (18,1%) patients, including 40 (50%) patients after PCA and 40 (50%) patients after LDC. Results. At control sonography kidney cysts were found in 32(80%) and 27(68%) patients after PCA and LDC respectively, but median cyst size decreased. Compared to PCA, LCD more often resulted in complete disappearance of cysts and also caused more prominent cyst size reduction (p = 0,027), however the lengths of stay (LOS) was longer after LDC (p = 0,005). After surgery the back pain and hypertension persisted in many patients, while the flow of urine from the kidney normalized in all patients. Conclusions. LDC compared to PCA was more effective in complete cyst removal and caused more significant decrease of cyst size. The LOS was longer after LDC, both LDC and PCA were effective in regaining urinary flow from the kidney. Back pain and hypertension should be considered relative indications for surgical treatment of kidney cysts. (For citation: Korneyev IA, Кiselev АО, Ishutin EJu, et al.
