Ñîâðåìåííûå òåõíîëîãèè ëå÷åíèÿ ðóáöîâûõ ñòðèêòóð aeåë÷íûõ ïðîòîêîâ Цель. Оценить современные тенденции в хирургии рубцовых стриктур желчных протоков на основе анализа отдаленных результатов различных вариантов вмешательств на желчных протоках, выполненных в крупном федеральном хирургическом центре за длительный период времени. Материал и методы. За период более 30 лет рассмотрен опыт хирургического лечения 349 пациентов с доброкачественными стриктурами желчных протоков. Восстановление желчеоттока посредством открытых операций произведено 301 (86%) пациенту. Антеградные чрескожные эндобилиарные вмешательства (чрескожная чреспеченочная холангиостомия, баллонная дилатация стриктур) выполнены 48 (14%) пациентам. Отдаленные результаты прослежены у 296 (85%) из 349 больных, из них у 258 больных с открытыми операциями и у 38 пациентов с эндобилиарными вмешательствами. Результаты. У 214 (83%) из 258 больных в отдаленном периоде получены хорошие результаты, заключавшиеся в отсутствии атак холангита. У 44 (17%) больных имел место рецидив стриктуры в сроки наблюдения более 20 лет. Эндобилиарная коррекция желчеоттока проводилась в сроки от 6 до 28 мес (в среднем 17 ± 10 мес). У 12 (25%) пациентов лечение закончено, признаков рецидива стриктуры нет (сроки наблюдения 4-24 мес). Заключение. Для разработки полноценной доктрины хирургического лечения рубцовых стриктур желчных протоков с учетом возможностей современных методов миниинвазивной коррекции желчеоттока необходимо дальнейшее накопление опыта применения эндобилиарных вмешательств, в том числе проведение контролируемых исследований. purpose of this study is to evaluate the results and role of radical operations in the management of hepatic hydatid disease. 317 patients with liver hydatid cysts underwent surgical treatment. Liver resection, total and subtotal pericystectomy were possible in 215 patients (67,8%). There were 115 patients (36,3%) with complicated hepatic hydatid cysts. Residual echinococcosis was indication for operation in 17%. Among all patients 53 (16,7%) presented other anatomic sites of parasite such as lung, peritoneum, kidney, and other viscera. The postoperative complication rate was 25,7% after operations without pericystectomy, 17,6% - after radical operations and 19,8% - after combined operations. The mortality rate was 1,6%. The recurrence rate was 6,7%. Removal of cystic and pericystic tissue and liver resection are the only type of operations for safe and radical treatment of liver hydatid cysts, especially in the cases of complicated and relapsing disease.
Surgery duration was 138±55.9 min. Blood loss was 69±112.7 ml (range 0-500 ml). Intraoperative complications (bleeding) were diagnosed in 3 (15%) patients. There were no conversions. Robot-assisted variant has been demonstrated in case of non-organ retroperitoneal tumors with diameter not more than 100-120 mm.
Aim. To improve the treatment outcomes, quality and life expectancy, prognosis in patients with hepatocellular carcinoma based on an analysis of treatment outcomes.Materials and methods. The analysis of the long-term results of treatment of 114 patients with hepatocellular carcinoma for 2015–2020 was carried out. Two groups of patients were distinguished: 41 (35.9%) patients were included in group I (a potentially resectable tumor with R0 surgery), and 63 (55.2%) patients were included in group II (advanced tumor does not suggest R0 resection).Results. Actuarial survival for patients with R0 surgery (48) was: 1 year – 85%, 3 years – 65%, 5 years – 55%; in patients with unresectable tumor after transarterial chemoembolization: 1 year – 65%, 3 years – 29%, 5 years – 11%; after local destruction methods – 0.5 years – 75%, 1 year – 36%, 1.5 years – 22%.Conclusions. The results of treatment of patients with hepatocellular carcinoma confirm the feasibility and effectiveness of a rational multidisciplinary approach. It allows you to achieve satisfactory results in multidisciplinary hospitals. The results are consistent with the data of large surgical centers.
