The World Gastroenterology Organization regards celiac disease as a chronic multicomponent autoimmune disease of the predominantly proximal small intestine, which develops with the use of products containing gluten. Celiac disease is difficult to diagnose due to the multisymptomatic course that can change throughout life. Celiac disease is inherited in an autosomal dominant manner with incomplete penetrance. There are new approaches to the treatment of celiac disease, based on some immune pathogenetic links. In particular, the ways of influencing the intraluminal digestion of gluten, the improvement of barrier functions, the effectiveness of immunomodulators are being studied. Patients should be advised to carefully check the contents of the ingredients before purchasing products.
The article is devoted to the analysis of the modern process of reorganization of healthcare institutions of the united territorial community (OTG) and the algorithm for creating a center of primary medical and sanitary care. The legal framework of medical reform and aspects of the activity of the primary medical care system of the OTG were studied. Modern approaches to the organization of a patientoriented model of primary care and the implementation of measures to improve medical and technical equipment and the financial distribution of funds are highlighted. It has been established that the creation of a primary health care center in financially independent communities will make it possible to fully embody a patient-oriented model in compliance with the development directions and principles of primary care in rural areas. The best is a clear division of labor between the economic part and the medical work. Promising is the conclusion of contracts in the framework of inter-municipal cooperation and civil-law relations with general practitioners-family medicine, who work as individual entrepreneurs.
The article emphasizes the priority of continuing education in a family doctor’s professional development and training. Current issues of continuing education as a strategic and prospective direction in postgraduate education development are indicated. The attention is paid to the impact of continuing education on doctor’s professional development in terms of World Federation of Medical Education. The development of postgraduate education according to the concept of higher medical education in Ukraine and the National Doctrine is also determined. The basic trends of study in Europe and Ukraine, major development directions, prospects and problems of professional education for family doctors are presented. Characteristic features of continuing education, major means and ways of implementation within postgraduate education are indicated. The attention of the medical community is drawn to the issues of continuing education in Ukraine. The existing system of family doctor’s training at the Department of Therapy and Family Medicine of Postgraduate Education as well as stages of organizational approach to the study of family doctors in the Carpathian region is analyzed.Ways to overcome the problems and the approach of continuing education to international standards are determined. It has been concluded that knowledge gained during continuous postgraduate education and medical practice as well as increase in motivation for continuous renewal and mastering practical skills obtained with experience will be the foundation of successful family doctor’s practice and decisive fac tor in the development of family medicine in general. There are many unsolved issues on the way to improve the system of postgraduate professional education in Ukraine and the Department of Therapy and Family Medicine. Therefore, permanent search for new advanced methods of family doctors’ professional development with the involve ment of the latest teaching methods and advanced technologies taking into account healthcare system features in the country in general and in the Carpathian region in particular in the period of its reforming is necessary.
Primary health care plays an important role in the fight against COVID-19 in Ukraine. The objective: is to analyze the activities of primary health care centers in the context of a coronavirus pandemic. Materials and methods. An analysis of the work of the Municipal non-profit company «Center for Primary Health Care of the Yamnitsa Village Council» from March 2020 to February 2021 was carried out. Results. Since March 2020, methods for diagnosing coronavirus disease have been actively introduced: PCR tests, rapid tests for detecting COVID antigen and detecting antibodies to coronavirus. Information on the number of PCR (+) patients, the number of suspicions, the number of patients who recovered, died are reported in the Internet resources of the community. A significant increase in the incidence of coronavirus infection in the Yamnitsa united territorial community was noted in October-November 2020 and in February 2021. Since the start of the pandemic in March 2021, 8 residents of the community have died from COVID-19. Treatment of patients with mild and moderate-severe forms of coronavirus disease is carried out on an outpatient basis. For such needs 5 oxygen concentrators for oxygen support of patients in day hospitals of outpatient clinics or FAPs were purchased from funds the local budget. This became relevant in the context of a sharp limitation of beds in hospitals in the region. Two sets of IDIS2GO telemedicine equipment were purchased at the expense of a targeted state subvention. Telemedical consultations regarding the tactics of managing patients with coronavirus disease and pneumonia are carried out with the pulmonologists of the Department of Therapy and Family Medicine of postgraduate education of Ivano-Frankivsk National Medical University. This allows to limit contacts, save time for the patient and the doctor, and maintain the quality and speed of medical care. Соnclusions. The COVID-19 pandemic has significantly changed the routine of primary care physicians. The load and the risk of infection have increased, but the reform system has not stopped, the experience has been enriched, the possibilities for providing assistance, and the introduction of information technologies have expanded.
