The aim: To research the effectiveness of cryopreserved blood plasma replacement therapy in patients with primary mannose binding lectin (MBL) deficiency, suffering from chronic active herpes virus infections. Materials and methods: Patients of the study group (SG) n= 36 additionally received cryopreserved blood plasma therapy Octaplas (Octapharma, Switzerland). Patients of the control group (CG) n=36 received only chemotherapy with Valganciclovir 450 mg 2/day per os for 1-3 months. The diagnosis of active herpes virus infection was established by PCR of blood leukocytes. Statistical analysis of the obtained information was processed by the calculation of the chi-square (χ2) Pearson criterion, the odds ratio and the associated 95% confidence interval (95% CI). Results: The adding cryopreserved blood plasma substitute to standard therapy with valganciclovir for the treatment of chronic active herpes virus infection in patients with total serum MBL deficiency below 50 ng/ml, allowed to get more negative PCR results. The effectiveness of combination therapy was 50% higher in carrier of HHV-6 (χ2=8,533 and р=0,004; Yeats correction 6,533 and significance 0,011; OR=11,667 and 95% CI=1,939-70,180) and 43% in carrier of HHV-7 (χ2=8,846 and р=0,003; Yeats correction 7,165 and significance 0,008; OR=6,375 and 95% CI=1,711-23,758), compared with monotherapy. The close association between deficit MBL compensation and the results of antiviral treatment is also reported. The effect of such treatment in patients with chronic EBV infection was less (27%). Conclusions: We assumed, that virostatic effect of valganciclovir is increased by MBL-mediated clearance of blood serum from viral particles.

In recent years, there has been a paradigm shift in the treatment of multiple sclerosis (MS) owing to the approval of a number of new drugs with very distinct mechanisms of action. All approved disease-modifying drugs primarily work directly on the immune system. However, the identification of an ‘optimal choice’ for individual patients with regard to treatment efficacy, treatment adherence and side-effect profile has become increasingly complex including conceptual as well as practical considerations. Similarly, there are peculiarities and specific requirements with regard to treatment monitoring, especially in relation to immunosuppression, the development of secondary immune-related complications, as well as the existence of drug-specific on- and off-target effects. Both classical immunosuppression and selective immune interventions generate a spectrum of potential therapy-related complications. This article provides a comprehensive overview of available immunotherapeutics for MS and their risks, detailing individual mechanisms of action and side-effect profiles. Furthermore, practical recommendations for patients treated with modern MS immunotherapeutics are provided.

Цель исследования: изучить эффективность заместительной терапии препаратом криоконсервированной плазмы крови человека у пациентов с первичным дефицитом МСЛ, страдающих хроническими активными герпесвирусными инфекциями. Материалы и методы. Для достижения поставленной цели проведено контролируемое нерандомизированное исследование на основании ретроспективного анализа клинических случаев. Пациенты исследуемой группы (ИГ) с хроническими активными герпесвирусными инфекциями получали комбинированную терапию валганцикловиром в дозе 450 мг дважды в сутки 3 месяца и препарат криоконсервированной плазмы крови в дозе 10 мл/кг массы тела в/в капельно дважды в месяц №6 (n=36). Пациенты контрольной группы (КГ) аналогичного гендерного и возрастного состава, также страдающие хроническими активными герпесвирусными инфекциями, лечились только при помощи валганцикловира (n=36). Диагноз первичного дефицита МСЛ подтверждали на основании обнаружения патогенных полиморфных вариантов промотора и структурных генов MBL2 по результатам ПЦР с рестрикцией. Сывороточную концентрацию МСЛ измеряли при помощи ELISA до начала терапии и на следующий день после каждого введения препарата плазмы. Тотальным дефицитом считали концентрацию МСЛ в сыворотке крови ниже уровня 50 нг/мл, а парциальным -от 450 до 50 нг/мл. Статистический анализ ассоциаций осуществляли при помощи расчета критерия хи квадрат (χ 2 ) Пирсона, показателя отношения шансов (OR) и 95% доверительного интервала (95% CI). Результаты исследования. Patients in the control group (CG) of the same gender and age composition, also suffering from chronic active herpes virus infections, were treated only with valganciclovir (n = 36). The diagnosis of primary MBL deficiency was confirmed on the basis of the detection of pathogenic polymorphic variants of promoter and structural genes of MBL2 according to the results of PCR with restriction. The serum concentration of MBL was measured using an ELISA before starting therapy and the day after each administration of a plasma preparation. The total deficiency was considered the concentration of MBL in serum below the level of 50 ng/ml, and partial -from 450 to 50 ng/ml.
