A Case of Xanthogranulomatous Pyelonephritis Associated with Renoduodenal Fistula

Abstract: Xanthogranulomatous pyelonephritis (XGP) is a rare form of pyelonephritis associated with repeated infection, chronic inflammation, and obstruction. Various fistulas, including those to the intestine, are a known association with XGP. Here, a 55-year-old woman with a history of multiple previous renal calculi presented with dysuria and back pain. Contrast-enhanced computed tomography (CT) revealed a soft tissue density in her renal pelvis and perirenal space consistent with XGP along with a tract connecting th… Show more

“…Xanthogranulomatous pyelonephritis (XGP) is a rare and severe form of chronic pyelonephritis characterized by destruction of the renal parenchyma and replacement by foamy, lipid-laden macrophages. 1 , 2 Although XGP is usually confined to the affected kidney, it occasionally extends to adjacent tissues, resulting in further complications such as perirenal abscess, adhesion to pancreas, or various forms of fistula. 1 , 3 , 4 Spontaneous pyeloduodenal fistula is an extremely rare complication of XGP.…”

“…Xanthogranulomatous pyelonephritis (XGP) is a rare form of pyelonephritis [1] . In majority of the cases, it is characterized by renal obstruction which induces repeated renal infection and inflammation which if not relieved and treated progresses to chronic renal inflammation.…”
